Since I'm garbage at StarCraft, pretty much everything I do is a mistake. Probably the worst one I've made is accidentally Psi Storming my own Zealots when my mouse slipped and put the cursor right over my army. =(
There are a few pretty "OMG"-worthy screwups I've seen in high-end games, though. Like blocking your own expansion while Forge fast-expanding, consistently forgetting to research Extended Thermal Lance. Stuff like that makes me cringe.
Really, just forgetting to start my lair in time and getting owned by Banshees/Void Rays. This only happens to me 1/20 games but it still makes me rage every time.
Oh and making 8 Drones when I see the guy's army is pushing to my base. ........... Stupid greedy Zerg.

My stupid noob mistakes:
1- Don't transition out of mutas. Yes, sometimes I don't. But i can still win. I'm silver. 2- Have over 9000 minerals and literally 0 gas. This has some kind of reason but I don't know it yet. 3- Not exploding burrowed banelings when a big group of marines are on it. 4- Not giving a shit to evo chamber upgrades. 5- Not spreading creep at all. 6- Getting supply blocked with 20 excess larvae before the 8th minute.

One thing I did that really helped with this was to play against the computer (or ladder if you want), do your normal thing and then check the replays. At the 10 minute mark you should have a MINIMUM of 30 SCV's (if you're Terran, the number is a bit different for other races but still a decent benchmark). Ideally you're 35+ workers.
For me, it's usually supply blocks or forgetting to macro. Whenever this happens, I usually try to go into qxc's practice map and just crank out the build over until I get it right.

I tend to forget important things at my house and only remember them when I'm already halfway to my bus stop. It forces me to sprint back home and then sprint to the bus stop. I've done this countless times with my wallet.
I forget zergling speed like every 5th game, it is the most annoying thing ever.

Also, I sometimes accidently hotkey my first 3 larvae instead of my hatchery at the start of the game. (split workers at start, click on hatchery and hit sd to make drone, then forget to click back to hatchery before ctrl+5) Usually it is no big deal, but sometimes I end up hitting 5sd without realizing it and boom, idle drone.

Actually spot a dark shrine building? MULE UP. See a stargate for a void ray? MARINE MARAUDER UP. See them with a starport and tech lab? MULE UP.
I actually play right and see what they are doing, then use my scans for mules and go "oh yeah! I forgot they were going cloaked stuff!" or play as I normally do instead of actually using the scouting info.
