Since I'm garbage at StarCraft, pretty much everything I do is a mistake. Probably the worst one I've made is accidentally Psi Storming my own Zealots when my mouse slipped and put the cursor right over my army. =(
There are a few pretty "OMG"-worthy screwups I've seen in high-end games, though. Like blocking your own expansion while Forge fast-expanding, consistently forgetting to research Extended Thermal Lance. Stuff like that makes me cringe.
Really, just forgetting to start my lair in time and getting owned by Banshees/Void Rays. This only happens to me 1/20 games but it still makes me rage every time.
Oh and making 8 Drones when I see the guy's army is pushing to my base. ........... Stupid greedy Zerg.
Hey, you're the guy who wrote about how to get ranked in the IT industry, it was a nice blog and among my bookmarks.
My stupid noob mistakes:
1- Don't transition out of mutas. Yes, sometimes I don't. But i can still win. I'm silver. 2- Have over 9000 minerals and literally 0 gas. This has some kind of reason but I don't know it yet. 3- Not exploding burrowed banelings when a big group of marines are on it. 4- Not giving a shit to evo chamber upgrades. 5- Not spreading creep at all. 6- Getting supply blocked with 20 excess larvae before the 8th minute.

One thing I did that really helped with this was to play against the computer (or ladder if you want), do your normal thing and then check the replays. At the 10 minute mark you should have a MINIMUM of 30 SCV's (if you're Terran, the number is a bit different for other races but still a decent benchmark). Ideally you're 35+ workers.
For me, it's usually supply blocks or forgetting to macro. Whenever this happens, I usually try to go into qxc's practice map and just crank out the build over until I get it right.

also i keep selecting multiple workers to build shit, and when i build stuff i always move towards the site before building, so pretty much half my workers go HEY LETS GO TO THAT RANDOM LOCATION. pretty much lose 100-200 minerals of mining time doing this =\

I tend to forget important things at my house and only remember them when I'm already halfway to my bus stop. It forces me to sprint back home and then sprint to the bus stop. I've done this countless times with my wallet.
I forget zergling speed like every 5th game, it is the most annoying thing ever.
"Hm, why does my army feel so slow...where are the wings...why don't my fucking zerglings have wings...ahhhhhhhhhhhhhhhhhhhh."
Edit:
Also, I sometimes accidently hotkey my first 3 larvae instead of my hatchery at the start of the game. (split workers at start, click on hatchery and hit sd to make drone, then forget to click back to hatchery before ctrl+5) Usually it is no big deal, but sometimes I end up hitting 5sd without realizing it and boom, idle drone.
Well, for sc2 the main mistake I make that makes me rage is when I actually scout cheese and don't respond. Often times I have 1 reaper to scout in tvt or tvp, and if I....
Actually spot a dark shrine building? MULE UP. See a stargate for a void ray? MARINE MARAUDER UP. See them with a starport and tech lab? MULE UP.
I actually play right and see what they are doing, then use my scans for mules and go "oh yeah! I forgot they were going cloaked stuff!" or play as I normally do instead of actually using the scouting info.
Quite often I make the mistakes of not adding gateways after I expand. Can't even count how many times I've had a game won and then he pushes out with a one-base army and steam rolls mine cause I can't macro enough units up in time. I even sometimes manage to crush their initial push, expand, prevent their expo, but still manage to lose cause I forget about those gateways.
- Forgetting to get upgrades - Staying on hatchery tech forever - Having 70 workers on 2 bases - Seeing a probe enter my base and hiding, and still losing to a cannon rush
I'm a very greedy player and almost always never spend all my money cause i can't keep up in production to my massive amount of minerals. I also get supply stuck very often
I honestly about 50% of games against Zerg forget that backrocks even EXIST until i see 20 speedling run into my mineral line. Then im like god im a F#ucking idiot sometimes. Or my personal favorite is when i rally workers NEXT to a mineral patch so im wodering why my income is so low and then is ee the little F1 picture with 6 workers all just chilling next to the minerals or one thts worst but similar is rallying workers to a worker so i have 6 probes and im wondering why my minerals are so low and why these probes look weird chasing this other probe....Oh damn it i really suck. hahahaha
- Forgetting Stim/Shell - Stopping workers during early spam - Chronoing before realizing i'm blocked - Relaying units into enemy armies - Accidentally adding buildings to my main army control group - Tech Lab instead of Reactor - Factory instead of Ghost Academy - (Zerg only for some reason) Pressing B instead of V and throwing myself off for about five seconds
Adding a few newly created mutalisks to my control group of mutas, which is across the map, and then issuing an order to the control group which ends up making the newly created mutas fly into certain death.

I've always thought it'd be cool if you could queue "meta" commands too. Like shift click the few mutas to move around the side of the map then once they get to that point, shift ctrl+add to control group.. if that makes any sense.
Chrono Boosting units that are supply blocked Forgetting 2nd gas Forgetting to put workers on 2nd gas Forgetting to make a cyber core Chronoboosting the 10th probe instead of the 11th Forgetting about upgrades until 12 minutes in DUAL TWILIGHT COUNCILS ARGHHHHHH
If i had a dollar for every overseer i forgot, i would be a rich man. Or, you know, if i just remembered to make one, i'd have quite a few more wins :[.

Haha I've actually done similar a few times where I accidentally build a supply depot instead of a starport, but that's just due to misclicking. Luckily (unluckily) I'm a slow enough player that I almost always see it right away.
When playing a new map I will often block my nexus with my pylon for my wallin PvZ. I almost always have approx 2 less supply than I should when I'm saving up for mutas. I will forget a key building in my build order, commonly the nexus in PvZ lol.
SC2: I try to constantly chrono, and often will chrono despite being supply blocked. I never get detection as zerg. Cannot get my mind over the whole overlord needs to become overseer thing. If I don't expand I will always have 50000 too many workers. D: Well, even if I do expand I'll end up with way too many.
chrono boosting buildings that arent doing anything. summoning mules right next to the minerals and thinking that they are mining, i forget about them forgetting extra rax/gates when expanding.
list of funny mistakes i've made 1: realizing 30 minutes into a game I literaly have zero upgrades from my forge while my opponent is at least at 2-2-2 2: trying to out micro an early push and accidentily a+clicking my own unit -_- 3: forgeting to HOLD POSITION with my zealot on my ramp and facepalming after 20 zerglings run by into my mineral line 4: wasting a chrono boost because I'm suppy blocked.

Sometimes when I go hatch first, I forget my to make my pool after =_=.
Then sometimes when my opponent does a drop and tries to snipe tech buildings, I forget to remake my spawning pool despite remaking everything else lol.
oh i have another one, forgetting whee i put my observer! I usually have them on a hotkey but sometimes i put units on that hotkey and then its impossible to find.
