You will be even more surprised to know that before modern medicine, about 50% of pregnancies resulted in the death of the mother and/or child.
Pregnancy is literally THE WORST STD.
PS- That 1/5 stat is higher the earlier in the gestation process. Most of it happens in the first trimester or even few weeks and a lot of women might not even realize they were pregnant so it's probably actually higher like the other guy suggested. Iirc it's more than 50% of them.

This is due to the expansion of the Brain earlier in our evolution. Evolution doesn't naturally solve every possible problem. Take bipedalism for example, our spines aren't perfectly suited to walking upright, which is why like 9/10 adults over the age of 35 have some form of osteophyte formation and/or other problems.
