Keep in mind that the end-mission cinematic has at least two nydus worms popping up inside your base. If you built somewhere where the nydus worms are supposed to pop up, the game destroys your buildings to make room for the nydus worm in the cinematic. Yes, that's right, you can still fail the achievement IN THE CINEMATIC after the mission's supposed to have ended. Don't put your buildings where the nydus worms are supposed to spawn.
Edit: This is mission 3 in the Mar Sara campaign. In fact, build as little as possible, just in case.
I played it again and I'm pretty sure that's what happend the first time. Silly!
The same thing happened to me :S :S :S.. now I have to do it again..

TLDR :p
But this is the way I did it.
I salvaged the two top most bunkers. Use the extra mineral to get the economy going and throw down another 3 barracks. You can rescue all the marines if you want. Pump out marines and medics until about 11 minutes left and then push out across the northern bridge. Destroy the two hatcheries in the northern part of the map and then retreat back slightly. The AI will rebuild the hatcheries and in a few minutes attack and destroy them again.

I had about 90 food worth of marines, and 16 food worth of medics. 5 minutes left, I attacked the bottom left base. The one with 3 hatcheries, I pulled my units back and fought the base-defending army w/out the spine crawlers. I brought the units back that were being pummeled by the hydralisks, and I lost about 20 food worht of marines. By the time I killed the 3 hatcheries, it had 2 minutes or so left. At this point, units were pouring out of the nydus, so when heading up to the top left base, I was getting constantly harrassed by zerglings/roaches. By the time I got there, I thought I could quickly snipe the hatchery, but I was wrong. I had probably 50 or so food of marines sniping it, but the mutalisks and the constant stream of units stopped me before I could snipe teh hatchery, I've tried doing this strategy a couple times, each failing in the same way. I've decided I need to attack earlier, so I've tried. The problem is I can't get enough marines to do anything, and the base defenses rape them. (50 food or so of marines) I have 4 barracks, one with the tech, like someone suggested, and I had the upgrades, and its on faster. I just can't do it. D: The other achievement was a cake walk. All you have to do is build 2 extra missle turrets in the front, and 1 on the left side. And have 2 extra bunkers on each side, and about 4 scv's on each side to repair the bunkers. Save it after every wave if you're scared, that way you can load if you're not watching and a couple roaches snipe one of your bunkers.

Also worthy of note, if you take the top left expo where there's 2 hatches (also least defended) and leave the drones alive, the drones will remake those hatches. Let them morph into new hatches and kill them immediately. bam, achievement.
I kept trying to break the bottom left expo to no avail b/c I'd wait too long to push out - between the forces already there and the nydus worm it never ended. Tried it like 4 times this way, then tried the method I outlined and got it first try.

omg that was annoying.... took about 15 attempts to do it, kept failing after getting the three lefthand hatches. eventually i got my micro and macro right and had a second force ready to bolster my battered main army when i went in to attack the final hatch.....
As above is salvaged my outside bunkers and fell back. Dropped a third rax with reactor, saturated my mineral line and then pumped marines and medics until i had about 140 foods worth of units total.
Then i struck out and took out all the spine crawlers just outside my base, left a few troops behind to stop any new ones going down and microed upto the three bases... remember to focus down enemies and just ignore the spine crawlers (let your medics do some work lol) once you get the first two hatches down you can take out the crawlers.
While you are doing all that keep pumping marines and medics and set them to rally just outside your base. Meet up with your main force as it leaves where the three hatches were, and go up the choke to the top left base, again focus everything down.... especially the air units (spalsh damage hurts!) and then just take out the base. done
