Same thing happend to me while playing on Normal. I protected all the buildings and didn't lose any and just didn't get the achievement at all. Pretty lame, could be a bug.

Notice that you have to beat the mission without losing or salvaging any buildings.
Maybe you salvaged your inner-most bunkers? If so, then it doesn't count.

In the mission "Zero Hour", there's an achievement (Hold the Line) where you have to survive without losing or salvaging any structures on Normal difficulty or higher.
Keep in mind that the end-mission cinematic has at least two nydus worms popping up inside your base. If you built somewhere where the nydus worms are supposed to pop up, the game destroys your buildings to make room for the nydus worm in the cinematic. Yes, that's right, you can still fail the achievement IN THE CINEMATIC after the mission's supposed to have ended. Don't put your buildings where the nydus worms are supposed to spawn.
Edit: This is mission 3 in the Mar Sara campaign. In fact, build as little as possible, just in case.

As several have said:
First: You CANNOT just get tech upgrades. When you go back, you play it as if you were that particular tech level. IE, what you had the first time is what you have now.
Second: This MUST be done on Hard difficulty to get the achievement. So, people saying "just play on casual"... no go.
Now, how did I do it?
I salvaged my front 2 bunkers, and put 4 guys in both of the bunkers on the second level, closest to my barracks. I put x2 cores on my Barracks. I just pumped Marines. I did it with Medics several times... could never get it. I'm sure others can, if not, try Marines only. I got just enough gas to give me the 2 upgrades for the Marines.
Other than that, lots of SCVs. Lots of Marines. Go rescue everyone you can. I immobilized my main group of troops after the second group appeared AND that left wave of zerg came. (Right after the call a wave from the left come, so let the bunkers do their thing, then go.) I think cleared the road of any zerg unit producing creep and any crawler up to the initial ramp up to the western zerg base. I held there and kept clearing out what came. Made sure no creep was added.
When the third group came, I pushed up the ramp, targeted the zerg units, then back to crawlers. Back and forth until I was through. Then, I took out the first base the the left. Killed all of the crawlers that remained while also disposing of the Zerg units. After that, just take out the hatcheries and move north to the next base. Rinse and Repeat.
I should say that about half way into my First assault on the western base I sent a group of Marines to reinforce, which was definitely needed if you use this technique. Before hitting the Northern base, I took the small group that was created at my base and moved them into my main Army. (They just held at the road)
Especially at the Northern base, there are a couple crawlers that you can just maneuver your guys around and just hit the hatchery and any Zerg units that come. That makes it fairly easy and less troublesome.

Yeah, most SC:BW was just that-
1) Turtle with MINIMAL forces 2) Econ hardcore, never stop building workers 3) Expand early as possible whie see 1). 3) Dual upgrades 4) Then make 15-20 gateways/rax/hatcheries, GG
Same applies to Warcrat 3 too. You just gotta have patience and reach critical mass (150-200 food). That's the biggest mistake of beginner RTS players. They send 50 food army 4 times and get them all killed when they could wait til 200 and rape all for the same money.
Same for SC2, OP. If you think you're dying, it's because you simply don't have enough units. If you're racing against time, you find the sweet spot to mass everything and finish it at once.

Well i really don´t see the problem with that achievment. There are only like 2 spinecrawlers 5 hydras and some mutas definding the hatches and you can just streamroll them with about 150 food of marine medic. The attack waves are pretty lame on hard just some lings and hydras. If you have some bunkers and patroling svcs all you need to do is like getting 3 rax and pumping a big bio ball and attack move.

To the people who suggested sacking those outer bunkers, thanks! I finally finished it using that strat. I put 2 reactors on the first two Rax, and got enough gas for both upgrades, then stopped getting gas. Just made a crap ton of 1/1 marines and decimated the SW zerg base with 3 hatches, then killed one of the north hatches for my 4th.
I almost died, from not having enough guys protecting my base, but you don't actually need to beat the mission if you just want this achievement. Thank god, I don't have to use the original portraits anymore.

I found it to be pretty easy, I simply walled off with three bunkers (with marines in) at the right side, a bunker (with marines in..again) and a barracks on the left side, then procceeded to macro up a forced of medic+marine. Keep making SCVs, having 3-4 at each choke really helps out with repairs etc. Just build up units, then walk right over the south western bases, kall hatcheries but let the drones live. They'll rebuild, kill another hatcery while they're rebuilding.

Man, for the "Hold the Line" achievement, I thought "no salvage" meant no repairing, so I went on deactivating auto-repair on all SCVs, building 6 bunkers at both chokes, tons of towers between them, and keeping 8 marines in front of the bunkers so they absorb damage. What was really a pain is that at some point in the mission, you are given an SCV that automatically goes to repair your bunkers, and I thought I had to catch it before he got there. Twice he did, and I restarted because of that. I'm such a noob. I didn't understand why I kept on failing the mission. Gotta be those towers in the middle being destroyed by the nydus in the cutscene. The last time, I skipped the cutscene, so that may be why I got it.

I struggled with that achieve initially too, but after reading what others did, I realized I was making it much more complicated than need be.
Here's what I ended up doing:
I salvaged my front two bunkers, built another rax, two reactors, one tech lab, a refinery, and pumped SCVs until saturated, all the while pumping rine/medic.
I save the first group in the NE corner pretty shortly after announced. Then around the 10 minute mark I take my M&M ball to the SW base with the 3 hatcheries. Change your rally points to near the base. Do some minor micro (not fighting units and spine crawlers simultaneously while they are uphill, for instance - just pull back and kill the units then run up).
After killing those 3 hatcheries, save the troops right up the ramp. Change your rally points and then proceed further NW to kill the last hatchery

I ended up just getting max food supply and then all-in'ing the computer.

I ended up doing it exactly in the way that EscPlan9 describes. Makes it pretty easy.

I got this achievement yesterday. I salvaged the innermost bunkers and did a fast bunker push just outside the left base. I had three bunkers at the right choke which easily defends that side with some scvs repairing. Then I slowly pushed until I could bunker right below the ramp of the left base. Then I lured the waves that gathered up on the ramp and got them killed at my bunkers and then rushed up the ramp with a large force of marines.
With one minute left I did an all in push of the top hatchery taking every single unit I had, including scvs and the marines defending my base. I was able to take out the hatchery before my units died. Meanwhile, my base was getting destroyed, but I lifted my CC away and it died literally to mutalisks one second after the countdown expired

Just macro up a giant ball, get the 1/1 upgrades save the people who need help and during the final save around 9 oclock go south. Kill two spine crawlers, rape the 3 hatcheries then back off and turn around again, by that time they have ordered drones to construct new hatcheries.
Destroy one of those, get achievement, go back win game.
As for the hold the line without destroying or salvaging bunkers, make two additional bunkers (one on each side), use marines to soak damage in front of bunkers and have svcs repairing.
