Too many factors to list them all.
The generic ones are your carries are able to do damage without dying or being unduly forced to attack tanks over priority targets. Positioning is usually the biggest one. You don't want to be in the middle of 9 people as the AD carry, but you don't want to be three screens over not attacking anything either.

Depends on... your role, your team comp, the enemy team comp, how kills are distributed, positioning, who engages who, etc. Generally, the main points are this:
- If you're a ranged damage dealer (mage or AD carry), your job is to do as much damage as possible while staying safe. The old WoW adage is "A dead Rogue does 0 dps", so position yourself where you're not taking damage and won't get hit by skillshots or anything. If you're in danger, run, don't stand and try to tank it unless they're like, already dead.
- If you're an assassin, wait until the fight starts (being in the beginning is just begging to get gibbed). Find the closest target you can burst and murder them, then get out as soon as you can. Wait until your CDs are back, then go back in and gib someone else.
- If you're a tank/bruiser, look at your kit. The heavier your defensive kit (WW's lifesteal, Galio's heal and ult, etc), the more you should just jump in and be unkillable. The more CC in your kit, you should focus on keeping enemies off your carries by peeling or locking down the enemy carry.
-Supports just do the support thing. Heal who's low, CC enemies away, etc.

I specifically like how MLG are using casters that aren't that well know. I'm enjoying them as long as they're not on screen

Like half the game has just been pause time. While it's partially their fault for getting DDOS'd, I don't think they should lose a qualifier just because of it.

I've been playing a lot of ad lately, and against a team of malph taric and cho I don't.. do damage :|. Unless my team peels for me for like 5 straight minutes no one on their side dies, and I can't get to their damage dealers without putting myself at risk of malph ult/slam/taric stun/cho etc. I don't know how to deal with an enemy team that just zergs me either (Olaf/Malph style champions).

Does 9.9AD from runes round up to 10? I remember this being discussed before but I forgot.

It only rounds to make it look pretty on your screen. In actuality, nothing rounds.
