What are your thoughts on BT vs IE as first ad carry item?
In my opinion you should always get BT first on Vayne, Graves and Ezreal because you need the survivability to fight from close range and you do a lot of vamp with Ez's Q or Vayne's W.
I have doubts on what to get on Caitlyn and Twitch. I used to get a vamp scepter for the early game and then go for the IE but in season 3 is so cheap to just upgrade the vamp scepter into BT and it boosts your mid game a lot . I don't think that it's worth to rush IE anymore unless you are really behind and you need to go for late game.

Depens on how the game plays out. Thister wisper warmogs is a dominating mid and early game build but as soon as you cmplete a ie and a dancer your dps starts jumping like crazy with the way crit works. Even on graves sometimes I go zeal ie first beacuse of that insae scaling.

Yes spell vamp works on Ez's Q . Not sure about Vayne though  .

Bloodthirster is IMO a terrible item on Vayne as it has basically no synergy with her kit. W only scales with attack speed and R scales with crit and attack speed. BT is strong for champs with easy engage-disengage mechanics that allow you to abuse lifesteal, but Vayne is a champ that goes HAM and wins fights, she is quite bad at short engages.
Phantom Dancer or Statikk Shiv (still unsure what I think of this item) are the ways to go for Vayne. Warmogs when you need a defense item provides all the sustain you need.

Spell Vamp does not care what kind of damage you do. If you press a button and it does damage, spell vamp works. Not sure about Ez q, though, since it is weird in general.

Just bought Draven, and I read the Champion thread and all of the appropriate advice, but I still can't quite figure out the Axe mechanic, or find a good video on it.
Short of watching Destiny (shudder) I'm at a bit of a loss.
(Also playing him requires such an aggressive playstyle that I'm not used to...ugh.)

Not quite. Spell vamp does not work on abilities that apply on-hit effects, instead those apply lifesteal.

But I kinda interpreted this as "so behind you're boned harder than a pornstar"

Accept you're behind, inform your jungler, buy lots of potions, avoid trading blows like the plague and focus exclusively on csing in order to not give out more kills. Imho, being behind as an adc is much less worse than being behind as top or even mid (in a lot of cases).

Have to be aware of different power curves and play to your strengths.
Say you're Corki with 4.5k gold vs Ezreal with 6k gold. You'll still shit on his face in a 1v1, so if he gets cocky because of his lead you can punish him. A kill, shove a few waves and take tower and suddenly you're even gold.
If you can't win the duels, then just farm as much as possible without dying and show up to skirmishes (don't get caught in your own jungle!).

I think it's worse to be behind as an adc imho, top and mid still can have a lot of utility with CC and stuff. An adc has to do damage and when he has less than his opponent it would be really hard to do as much as him in a teamfight.
