No the out-of-sync issue it is not happening every game, but quite often. Last pages of this thread contain several times the answer to your question. Many previous answers are much more detailed than this.

This happens when I start SC2gears. Haven't been able to get it to work after a MM game. The last game that I have is from 02/28/15 around 7 PM EST. I haven't been able to get a game working since 02/29/2015.
Having this issue for a few days now. Any updates?? I'm guessing that something with the DB files needs to be updated.

The new DB unlocks new MMR tool data (good games are possible once again). Based on the web profile polling results NA, KR, SEA and CN profiles give stable readings (no errors regarding bonus pool). But for EU server there has been 3 incorrect readings during today. But as there has also been several hundred correct readings the error ratio is very small and thus also EU data is unlocked even if very small percentage of fetches return incorrect bonus pool.
Saw this above, so the 'Version checker failed' is expected. Now I have no clue why SC2Gears isn't working.
EDIT: I changed a bunch of settings, reapplied the DB file (which wasn't a problem), restarted my comp, and it has been working.

another question (which i suppose many have asked ) i just got my account on sc2replaystats created and want to use it with scelights auto upload plugin. in general i use scelight for analyzing my replays. it is a pain to have to use them both (scelight and scegears). is there a chance of porting mmr tool to scelight ?

MMR tool is non-profit tool for the community. Scelight has a paywall that is problematic regarding this. Without it the port would likely have been done a long ago. The data quality is very important for the MMR tool (more complete data makes it easier to calculate offsets when they change). With Scelight user would feel like forced to pay and/or would forget to start the tool after waiting 15 seconds required to be able to push the start button (it should be automatic for all, not just for paying customers). In the past it has been a problem even with sc2gears that users simply forget to start it. With Scelight this (forgetting to start the tool after waiting 15 sec) would increase and thus more data would be lost. Also it would lead more work for me to support two separate replay parsers.
There are also lots of other features missing from Scelight that I personally like such as EAPM, ability to change where the replays are stored (if one tries to change the default location in Scelight, it is always reset back to the default), ability to check building placements after the match (one of the best features of sc2gears is ability to quickly review opponents building placements after the match without watching the replay. This feature is not completely accurate because building cancellations are not considered, but very helpful when you e.g. want to know why you missed opponent's key buildings and misread his build). There are other things too.
But some day in the future the replay format may change to make sc2gears unable to parse new replays. When that day comes a new replay parser is needed for the MMR tool. Scelight is one option, but there are other parsers too that may be suitable. Also Blizzard might change the ladder system considerably for LotV. It may even be that MMR calculations are not possible at all for LotV (or require considerable work effort to 'reverse engineer' how the system works to be able to provide accurate calculations). The release date of LotV is unknown, but its beta should be released during the first half of 2015. At this point we don't know if there is a future for the MMR tool after LotV. Time will tell...

2015-03-06: Blizzard was supposed to change league offsets / thresholds last night: http://us.battle.net/sc2/en/forum/topic/16542954341 But there is pretty much no change in nios.kr data in any server (the changes follow previous trends, amount of players who have completed the placement increases in a trend + leagues increase also in old trend). This makes me wonder if 1) the offset / threshold change has failed, 2) changes have been very minimal, 3) is nios malfuntioning, 4) are leagues locked, 5) the changes were something else than offset / threshold changes even if Blizzard claimed them to be offset / threshold changes.
The two previous offset changes happened: 2014-01-24: HotS offsets #3 (21 days after start of season 17, 2 days after patch 2.1) 2014-05-01: HotS offsets #4 (17 days after start of season 18) Both times the changes were reflected almost immediately in nios.kr data (took only some tens of minutes after the change until promotion spikes were visible). It is worth to notice that on January 2014 Blizzard claimed they made offset changes when patch 2.1 was released, but there was no changes. The actual changes went live two days later.
But if there was / will be offset / threshold changes then MMR calculation results for new MMR tool data will become incorrect until new offsets have been solved (but will be automatically fixed when the new offsets are known). To solve the offset it requires huge amount of user data (usually takes several weeks to collect). This time it will be problematic. The MMR tool server is still down. And it is starting to seem that we will have to switch the server (requires a new MMR tool version that would require manual installation). This would cause that the user base would become split and less data would be collected. I expect that considerable portion of the users do not actively follow this TL thread and will not notice if a new MMR tool version that requires manual installation is released. Thus even after potential server switch it potentially takes a long time to collect data before the new offsets can be solved (much longer than normally). But if the server switch is made, you will be informed in this thread.

I plan to add a SSD to my computer with a fresh install of Windows. Therefore, I will have to reinstall SC2gears along with the MMR Plugin.
Will I be able to transfer the data (i.e. the MMR history) from my current HDD to the new SSD? If so, what file do I copy over? I want to note that I'm not using the SC2Gears Database, if that helps.

Go to '[PATH_TO]\Sc2gears\User content\Plugin file cache\' and copy entire 'mmr.plugin.MMRPlugin' directory into corresponding directory of your new installation (it contains all the MMR tool data and settings). Thought if you continue to use your old hard drive after the SSD installation, you can drive sc2gears from there too (sc2gears can be located anywhere).
Or actually you could copy your entire Sc2gears directory into your new SSD and use it from there (all your old sc2gears related settings also remain, sc2gears related settings are stored under '[PATH_TO]\Sc2gears\User content\' directory). But remember to use one instance of sc2gears after that (data is saved only under that sc2gears directory structure)
The 'SC2Gears Database' has nothing to do with MMR tool. Thus it does not matter.
--- I will be releasing a new DB file that requires manual installation tomorrow Sunday due league lock (locks data as final bonus points are not known). Another DB file will hopefully be released on Monday after the final bonus points are known (releases lock).

But based on nios.kr there has been only minimal changes (potentially the thresholds/offsets for lower leagues slightly increased). With minimal changes the old MMR tool values should be almost accurate. For example last time May 2014 when they made changes, the changes affected mostly diamond and and master regarding offsets. And many other offsets remained the same or changes were ~20 MMR points. And as one match on average equals 16 MMR points, most lower league users did not see pretty much any difference.

2015-03-29: DB file (v 21.2) for start of the ladder lock period is available. It requires MANUAL INSTALLATION! It should be applied after your home server has been locked
Ladder season lock is supposed to start at local times at midnight (2015-03-30) on each server. The new DB file should be applied after your home server has been locked (when writing this KR, CN and SEA should already be locked). This DB locks new data (no good games possible). When final bonus pool for the season is known for each server a new DB that unlocks the data will be released (hopefully during Monday).

Your current DB file version is shown at the top of the main log of the MMR tool. You can install the new DB by unzipping the downloaded packet to following folder: [PATH_TO]\Sc2gears\User content\Plugin file cache\mmr.plugin.MMRPlugin\

As the MMR tool server has been down, I haven't been able to check people's profiles. Blizzard supposedly changed some of the league offsets recently (based on nios.kr the changes were minimal). And based on your picture the readings are still very consistent. And also based on the picture seems that at least gold-platinum offset has not changed at all (or very small change).
