You left out RNG! "oh, you found a nice item, I'll throw 3 orc priests smiting you and a centaur at lvl 2 of the dungeon just for lulz!"
RNG not only kills you, he steals your will to live as whenever you do fine, it will be there waiting, dropping you down a shaft into a pack of centaurs or a vault with a zillion killer bees.
It seems that the most OP unit in the game is the player character in all the modern games... I miss the times of feeling weak and miserable and going against impossible odds.
few page ago someone posted about Absolute Virtue from Final Fantasy 11, that boss and Pandemonium Warden, still from ff11, has to be the ultimate OP units. In order to fight Absolute Virtue you had to pop and defeat Jailer of Love, that boss alone, at first, was very hard, and by hard i dont mean like a wow hard boss, a successful attempt was easily 1:30 to 2hr. And then after victory you had a chance for AV to pop. It was considered foolish to attempt this w/o 25-35 extremelly geared and skilled ppl. It litterally took years to have a 1st legit kill on that boss, and on top of that on the very 1st kill, it dropped 0 loot, wich wasnt a bug at the time, every boss could drop nothing. Linkshell ( = guild ) used to spend hours trying to get it down when it was "possible" but at any second it could be an instant wipe or the boss could be at 1% and perform the white mage 2hr ability and heal back to 100%.
AV was the last boss of the 2nd expension, the killable nerf happened when ppl where trying to kill the last boss of the 3rd expension wich was Pandemonium Warden, a boss that was as powerfull, to give you an idea how that boss was, a linkshell spent 18 real life hours doing 1 attemps, and they didnt kill it, they didnt wipe, they just gave up.
Also, the 1st strat that allowed ppl to kill those boss was by using alot of dark night with top gear and a multi-hit mace, the mace alone was worth 120 Million gil ( gil = gold) when i quit the game, to give you an idea how big of an item it was, i sold all my gear for 125ish million gil and sold the gil on the web for 2,5K USD, so that was how much the mace was worth. As a group you needed several of them + all the gear to support it, just to have a slim chance to defeat the most OP units in a game that was ever created. Keep in mind that boss was undefeated for years till Square Enix nerfed him, cant be more OP then that.
In my opinion, the concept of quad damage (Quake Series) is the most "broken" thing in the world. In the hands of a quake master, it's literally unstoppable.
Anyone remember Clanbase Eurocup 3 Video. In the Beginning SteLam gets quad + plasmagun and rapes the shit out of everyone.
Jazz from eternal sonata. He hits like a truck and hits twice with every swing. He gets insane damage boost from items and team abilities. If you stack your combo properly you can kill anything in the game almost immediately.

KV with the KV-2 turret and the 152mm "derp" cannon slugging high explosive rounds... one hit kill doom to low and equal tier tanks, potential for crippling damage to higher tier tanks :D
