I know how to use control groups and add units to them with ctrl + # or shift + #, or remove them by shift clicking the unit and reassigning the control group. That is not the issue that I am having.
Randomly in the game, some of my control groups have been completely clearing out. For instance, I set my nexus to control group 3. At some random point throughout the game a couple of times now, that control group is empty, despite the fact that my nexus is still alive.
Is anyone else having issues with this? Has anyone else seen or heard about control groups just randomly disappearing in the middle of the game without any reassignment taking place? No units have taken the place of these control groups. I had ALL of my control groups right around 3 minutes disappear one game. I am going to check replays and see if I can see the groups disappear on there as well.

The only thing I can think of would be pressing the wrong buttons. Like try to make group 2 with stuff but accidently press 2 and 3 at the same time, soon after that group dies so it looks like your nexus group (3) got wiped out.
Other than that, I haven't heard of any bug concerning this lately.

Do replays still have the control group information in them, if so next time it happes watch the replay carefully.

You understand the difference in Ctrl+# and Shift+# right? Ctrl assigns the group to whatever is selected. Shift adds whatever is selected to the group.
Watch the replay with player camera on and make sure to Clear Selection (esc) so you can see what your selecting. You should be able to tell exactly what happened.

If you right click on the little "tab" that shows your control group, it will reassign whatever your current selection is to that control group. This may/may not be your issue, but worth a thought 
(You can disable this in the options)

hes talking about something else thiis has happened many times to me wwhere my hotkeys just get randomly wiped

The most likely culprit is that you accidentally reassigned a unit or units to that group and they died. As stated earlier, watch the replay and see what happened.

This is actually the most like culprit. As I used to have this problem and figured it out rather quickly. You can disable it in options as stated above.

Above your user interface shows your control groups and what units are assigned to what, and if you click on it it will replace whatever you have selected to the control group. Go to options and make the control group bar unclickable and your problem should be solved.
