If i choose scaling cdr runes for 20% at lvl 18. Combined with 5% cooldown reduction from masteries. Then i have 25 % CDR.
I heard it is capped at 40%.
Therefore if i take one item with 20% CDR i will be at 40 ?
Am i right ?
Should i remove the cdr in masteries if i plan to buy a 20% cdr item early ?

You're right, you're capped at 40% CDR.

This is up to you. With your plan, you won't get max cdr before level 18 which will always take a lot of time. If you take the 5% cdr from the masteries, you'll be at 40% much earlier in the game, at the expense of wasted stats later.

I'm (relatively) new to league... switched from dota 2 - I've really enjoyed playing AP mids and have had success playing orianna/lux/zyra/cass exclusively.

1) Are there any really good guides out there for general mid play / general AP mid play?
2) Based on my strong preferences (I like all four of the aforementioned a lot), what other champs should I consider to ply at this point in time?

1) Not that I know of but I don't look for guides very often. Most champions-specific guides will have sections about playstyle and laning.

2) As a big Lux/Ori player, let me say that you will probably really, really like Ziggs, and possibly AP mid Lulu.

Junglers usually gank after red buff; when they take red buff depends on the jungler but it's usually around 3:30 i think? not sure about the exact number. Put down a ward in the mid bushes on red buff side and stick closer to the warded side and you should be safe. Guaranteed effective up through gold.

When the buffs first spawn, look at top and bot lane and see who's there. The lane that isn't there until around 2:05 will be the one that leashed for their jungler, so you know where he started. He'll then do a small camp and then his second buff. Depending on who their jungler is, he'll finish the buff at a little after 3:00, so you just have to be aware that he could come at you from that side at that time.

There aren't any more timings for the next few minutes, so it's mostly about if you can see him on the map (if he tries to gank a lane or walks over a ward), and how your opponent in lane is acting. If he suddenly starts being really aggressive for no apparent reason or just acting really different, there's a good chance he's trying to set up a gank.

The only other timing to keep in mind is that at around 7:05 the first buff will respawn. At low elo I think that people aren't really aware of these timings so much and they might not even notice that the buff's back up, but when they do get it, that'll often be when they hit level 6, which is very important for a lot of junglers (Pantheon, Warwick, Wukong, etc).

One other thing that might help is trying to get into the jungler's head, which means having a basic understanding of what the jungler might be doing at any given time.*

1) Is it a farm jungler with shit ganks pre-6 or an early game jungler who really wants to get early kills?

2) If you were the enemy jungler, where would you gank?*
-If you are a squishy no escape champ (like the ones you listed) and you are pushed up towards enemy tower while the other two lanes are not, the enemy jungler might come in for a gank attempt if he's close by. the longer you are pushed up, the more likely the enemy jungler will come to gank in my experience
-If you are dumpstering your lane opponent, it's possible your lane opponent is whining to the enemy jungler for a gank. you might get your lane camped by the jungler
-If you burned your summoners, your lane opponent might tell the jungler that your flash/barrier etc is down, and because you play squishy no escape champs, the enemy jungler might come in for a gank attempt
-If you are against a duo queue where one is the midlaner and one is the jungler, you might get your lane camped by the jungler so his buddy does well. This might apply even more to normals with 3-4 man teams; the jungler will most likely ignore the soloQ laner and help his buddies win instead

Most of this shit can apply to other lanes too where if conditions are met. For example, if your Vayne is somehow winning botlane, the enemy jungler might say "fuck that" and camp Vayne's lane, which makes it less likely that you'll get ganked.

*depends on skill level of jungler; some low elo junglers might just farm 24/7 and never gank and others might do really strange things like 100% camp a lane or L2 cheese gank or some shit or show up to a lane really late after a fight is mostly over. as you climb ranks enemy junglers will get better at counterganking/abusing power spikes/showing up to fights at exactly the right time
*also depends on your midlane matchup. some lanes are much harder to gank for than others. if you play a high mobility champ, you are harder to gank. if the enemy laner is nidalee or some other laner with low CC, the jungler might say "fuck it not ganking mid" and mostly ignore midlane. if your enemy laner is really bad and got his jungler killed once or twice, enemy jungler might say "fuck you are bad, not ganking your lane ever again" and let your midlaner continue to get dumpstered

Why does hexdrinker seem to be core in the pro's builds of jungle Kha'zix and Eve (maybe others?). It seems like a situational item to me, but according to Kobe and several recent pro games I've watched it seems standard. Is it just really cost efficient and a rare combo of AD + defensive stats? or is there something more?

A gank right after redbuff comes at about 3:10 for most junglers, 3:00 for the fastest ones (eve etc.)

My gut is telling me that they build this item because they're either not using mres blues or because they're diving for AP champs a lot.

The shield works really well with kha's ult, as the 50% damage reduction also applies to the shield, so with the current kha style of diving in, absorbing all the cooldowns, then running around the fight surviving through his ults tankiness, the shield is very strong.

Basically has it's amount doubled due to his ult, so a 500 hp shield, plus the mres is useful as their ap will generally be desperately trying to burst you down if you're initiating like they often do in pro play.

Also some teams run those comps where they try center their teamfight around getting a ton of shields/heals onto kha, (lulu/janna/nidalee/etc comps,) so he can be totally unkillable with his ult and run around getting resets, and hexdrinker helps that too by adding an extra shield, as well as some resists to beef up his other shields.

Eve I dunno, people (or well, diamond) have been going ad eve for a while now, don't know enough about her to have an opinion on it though.

Hexdrinker is really cost-efficient, like The Brutalizer, and to a lesser extent Haunting Guise. Since the junglers want quick patches early game to follow the solo laners' curves, especially in high level/pro play where they're less likely to get fed, it pretty much covers your MR needs till the late midgame, despite its slot-inefficiency later on, allowing you to funnel your farm toward physical defenses or damage.
Plus what Killerdoge said about shields and Kha.
Cant take LMS hipsters serious.
