Hence the name A-E-T-N 
1. You can use autocast easily with the keyboard without pressing Alt like the default keys 2. The item hotkeys use the same visual configuration as what is shown on-screen 3. You still use A, S and H like you do in Starcraft 4. You can instantly go Q-E, Q-R, Q-T, Q-N when you level up. Like right when you reach level 6, you can go Q-N-N-Click and cast your Ultimate. 5. You can shop fast by just pressing space 6. You can bind dominated creeps right beside the creep ability. 3-E-Click, and you cast the creep ability, then press 4 and go back to controlling your hero.

Abilities: QWER DF
Control groups 1-5. courier:F1, grab items:F2, Deliver items:F3 Speed boost:F4. So I can just roll my fingers over the keys :D
A=Attack,S=stop H=hold position.
Item Keys: Item1->CapsLock Item2->Space Item3->Mouse button 4 Item4->G Item5->V (these two normally go for passive stuff..dont like these last two keys too much..might change). Item6->Mouse button 5
Havent really worked it out completely..but yeah thats what I use currently.Also, the caps lock is quite comfy for me, I put it to something that I use regularly...but when have to type..people think your shouting :D

I have used the majority of this setup since Warkeys was released. I used to curl my thumb under my hand to rest on Alt and Spacebar to display health bars before the functionality was implemented. I also used control groups 1 through 5 for each shop.
I don't really see why you would ever need to auto cast an ability. Not sure why so many people have it bound. Maybe for Orb effects but those are normally either always on or you are using them every few attacks in which case I would be orb walking anyways.

1-0 - Control groups are all the top row number keys, i generally bind hero to every group at start of game (unless i play lone druid or something, then 1 druid 2 bear 3 both) F1 - courier F3 - shop Z X C ~ Space T - items QWERDF - skills G - ability learn side button on mouse - voice chat
i have courier and other units mirror the hero controls

I basically put courier/shop/level up buttons far away to prevent accidentally pressing them. No need for control groups with F1/F2/Ctrl hotkeys.

The rest are default I think. I used to use F1-4 for items, but someone in the TL chat room convinced me to try alt+qwer; so far, the alt items are working fine although I have pinged the map by accident quite a bit when firing off midas. The oddball control groups are for meepo so that ` is all my clones (blink/lothars poofing), 1-5 are individual meepos in order. (the T for attack is just because i got used to that via grid in sc2)

Unit Action:
I have auto-attack off. Attack = A Stop = S Hold position = H No move key since it's completely useless, especially with auto-attack off.
Select hero = 1 Select courier = 2 Select all other units = 3
Control groups 1-6 = F1-F6 Control groups 7-10 = 7-0 Control group Tab = TAB
I also have the ctrl to command all units option on. No other keys here, don't need them.
Abilities:
I use legacy keys. The only hero I switch keys for is Rubick with Ability 1 = G, Ability 2 = W, Ability 3 = N, Ability 4 = R, and Ability Ultimate = T. No other keys for Heroes.
For courier the only keys I have are Y, T, and R for Retrieve items, Transfer items, and Speed Burst, respectively.
Items: Z, X, C, Q, 4, space If a key is taken then I switch it to W or whatever is available. For Invoker I use alt+Q, and etc.
I've been thinking about switching my keys around to accommodate micro heroes like Chen and such; that's a work in progress.

QWERDF for skills, although I use Legacy for Nyx, Invoker and Puck, and possibly for other unreleased DotA heroes if I'm more comfortable with Legacy on them.
Alt + Q/W/E (top 3), Space and Alt + S/D (bottom 3) for inventory
Y for learning skills, I just leave it unassigned if on Invoker (hello ctrl-QWER)
B for sticky-buy or w/e it's called for TP scrolls, Attack/Stop/Move/Hold are default and I think that's it...?

Full disclosure: I'm a total Dota noob and have only just started playing.
I use a Razer Nostromo gaming keypad (which I love to bits), and have set up the layout shown.

Notes:
+ Show Spoiler +- ALT + Q,W,E,A,S,D correspond to the item slots, but I also bind mousewheel up and down, and two mouse side buttons, to item slots (2,3,5,6) for quick use. I try to be logical about the placing of these, e.g. travel items such as TPs go in mwheel down, Combat activations such as manta style go in Buitton4, "Panic" items such as Wands or Mekanism go in Button5. - The directional pad is used for unit shortcuts F1-3 and left is plain number 4, since I use that to bind a control group if I need to (rarely, since I'm still learning and have stayed away from micro-heavy multi-unit heroes so far) - On the same topic, it's not easy to tab through active selected units with this set-up, since I only realised this was a thing after creating the scheme  . I may need to put tab in a better place even for my helm of the dominator unit skills soon.

@Beard of Bees: Nothing wrong with the Nostromo bro, I used one for years to play MMO's (when it was still made by Belkin before Razer bought the design), and still pull it out every now and again. If it works for you, use it! It's a neat device that I don't think got enough recognition over the years.
That said, I use a mechanical keyboard for DotA, as the key reflex of a true mechanical is just hard to walk away from after playing StarCraft on one for years.
My layout is: Q, W, E, R, D, F for skills as usual 1: Select hero Alt + 1: Select all other units 2-4: Control groups T,G,B,V,5,6: Items (in that order, left to right, descending) ~: Select courier Alt+~: Courier deliver items
Part of my layout is obviously inspired by playing StarCraft for years, and being very used to tapping 1-6, and generally keeping my hand in that area of the keyboard.

As ex-HoN player here. I use basically standard HoN keys:
QWERDF: abilities 1: Hero 2,3: Control groups (more if needed) Alt+QWEASD: Items A,S,H: Attack, Stop, Hold T: Push-to-talk B: Shop C: Courier
I should really add some hotkeys to my mouse, but I'm not used to that.

The controls in HoN are really well done. I say that because they closely match mine, and the transition to HoN was so smooth for me because of this. Most players with really "crazy" setups, made their setups when the ability to change hotkeys in Warcraft 3 first came out. You used to be stuck with really weird ability hotkeys like B for blink on a hero that might have like W and O as binds as well. It made for really weird hand positions that you just had to get used to for every hero. You also had to use the numpad for item use. On top of that you had to press and hold Alt any time you wanted to display health bars. So you had a finger devoted to Alt.
When Warkeys and Dotakeys came out and you could remap the keyboard it was a lot of work to change everything, so a lot of players just fixed a few things here or there that bothered them. This led to really weird binds that people became accustomed to. Also some people just feel more comfortable hitting different keys. For instance in Diablo 3 and Torchlight I play with my hand on the right side of the keyboard using 90-= as my main abilities with [ ] as my pots. In WoW I played with 4567890-=TYUIOP as my main keys, but still homed my hand on the left side of the keyboard. Its really just personal preference and where you hand feels most comfortable on the keyboard.

1-4 Control Groups. If I was playing only dota, I'd probably not use more than 2 or max 3 control groups, but I play SC2 regularly, so I gotta compensate for that. I usually just have my hero on all 4, with F2 for courier. I sometimes have a personal bottle crow, in which case I can hotkey that separately. Forge spirits, spirit bear get their own control group. Naga images as well, but I don't play naga so =)

For most heroes i use QWER for the skills, only need to switch to Legacy key when i use Slardar.
Items: Alt+QWE ASD.
Courier binds to 2
Manually bind group to 1 when i use chen/ench and even meepo!
Level up = Space.
Shop = P
Scoreboard = [
