So, in reality she has 2 spells and an ult during a fight (you can spam Q of course), so that ult needs to be ridiculously strong, like Tibbers strong or Curse of the Sad Mummy strong, but since it is not close to that level of CD, it being that strong would break her for laning.
With max CDR and good Q aim you can get at least 2-3 Mantras off in a fight, more if it extends out for a long time. The upcoming changes will decrease the need for max CDR at that, as well. A quick fight goes like this: Open with RQ in an AoE comp or RE if you have a hard initiate -> Land a couple Qs that hit multiple targets -> W the diving assassin/bruiser (as each tick reduces the CD) -> then Mantra cleanup (RE for chase/run, RW if you're low and alone, RQ for finishing off people). Longer fights are even better.

At least in my experience, teamfights do not typically last that long. Sometimes I get a 2nd mantra off during the cleanup phase of the fight. People are rightfully afraid of AOE ults (most of my games tend to have them) so they rarely are clustered in the kind of way that would make RQ a scary ability. Currently it can deter divers a little for me. But Karma is IMO mostly strong for her slows/snare anyways and the empowered abilities don't really augment that significantly enough.

The lore changes they have done to Morde, Karth, and now Trundle and Olaf have been pretty depressing. They could have revealed these champion's connections whilst keeping former, more interesting, lore intact. I am disappointed that this is the direction they are going.
