Honestly I think Tiamat could be easily fixable to be a useful niche item, with some alterations to the cleave. The item is cost-effective for stats, it's just that the cleave is designed in such a way that it's downright worthless. First is that the cleave shape needs to be different. A circular cleave around the target is downright useless when the item specifically is more slated toward melee users because a lot of the cleave AoE will just be on/around the champion model. Even if it had the exact same AoE, a cone shaped cleave that splashes forward from the attack target would be a LOT more usable than the current circular shape. The second is that the AoE just needs to be bigger. You can trade off for a smaller cleave %, but the AoE right now is too small. This isn't even necessarily for its combat effectiveness--part of the ostensible usefulness of an AoE splash item is going to be as a farming/pushing item, and if you can't get the splash to hit multiple creeps if they shift around a bit, you're going to have a rough time using it.
If they did some of those changes to Tiamat I'd be pretty happy. It's already hilarious to build on champions like Fiora but if they made it to where the item's splash was wider and extended forward instead of in a circle I would probably buy it every game.
I dunno, there's so few melees who would care about getting that item that I don't think it could hurt much to increase/improve the AoE on it.

Yeah Fiora ult strikes five times and applies on-hits. It hurts a lot if the enemy clumps and you have tiamat but rarely do they clump well enough for tiamat to be worth it so it's still a really troll item.

Tiamat Shyv does a lot more damage than tiamat fiora. Vs a clump of 5 champs one shyv dragon Q scales the same as two Fiora ultis. I haven't done the math for new Twitch ultimate but it's similar(ly fun).

TL B once attempted a Tiamat Shyv + Ori combo, way back before Diana's release.
It turned out that Soniv sucked at Orianna, and I sucked at Shyvana.

Tiamat also sucks, so you have a lot of synergy there.
