People have stopped instabanning nid vs hotshot like a year ago lol. Hotshot haven't picked nid since what, the talon patch? That was when her heal got massively nerfed.
The reason whyap nid was so strong at the time is because aside from the fact that she can afk split push all day and win top lane for free, she wrecks the shit out of both the tank spam and poke comps that everyone was running at the time due to her godlike pokes and heal. Incidentally, she fits extremely well into CLG's trademark playstyle of just afk pushing and laming the shit out of other teams. That couples with the fact that hotshot was just so good as her makes her an instaban for any team playing clg.
Now hotshot can be as good with her as he wants, no one wants to ban her now since ap nid is worthless and there are so many better choice for a bruiser. She still wins lane, sure, but she contributes little to the team and her scaling is just garbage now.

I think it depends on the code, for example the Razor rune codes could be used on EU and NA whereas the Arctic Warfare Caitlyn codes could only be used on NA (correct me if im wrong).

Pretty sure the PAX codes will be locked to NA, just like the Gamescom codes were locked the EU servers. Since Riot started selling the promotional skins in their store they have a pretty big financial incentive to ensure that as few people as possible get the skins for free. Just speculating though.

Was only once that I saw, but he played Nidalee when Saint was still on clg vs Dig at IPL4 iirc. They ran their poke comp with Cait, AP Kog and Nidalee.
But yeah, he'll never play her again so long as he jungles. Unless he finds a way to make jungle Nidalee work.

I am so frustrated.
What should I do to become more map aware? I obviously know that I need to periodically look at the minimap, but I always find myself forgetting sometimes, especially when I am trying to make a play of some sort or distracted for some reason. This has been a problem to me since my early WC3 days, but WC3 wasn't really punishing on map awareness so this got slipped through.
What should I do to train myself so I can consistently look at the minimap? Any suggestions?

Well, I dunno, I still think Hotshot and Jiji are two of the best players to ever play the game, regardless of their current skill levels. Just because they were known for x champ and they don't play them anymore doesn't mean anything to me, really.

Make it a point to always ping when you see a jungler on the minimap and when someone leaves a lane.
Also, jungling improves map awareness like 10x.

a) just do it b) make a loop that reminds you every 3 seconds.
