That's not a gameplay issue, it's an issue with that particular beatmap. Generally those drain issues are resolved before the maps are ranked but not always.
Edit: Ah nevermind I know what you're talking about now that I see you compared it to Stepmania.
You shouldn't have any problem with dying to HP drain if you're playing the map well enough though.

I tried the game and i find core of it fun, but there are some things that annoy the crap out of me. 1.) In evrey game some anime chick pops out of nowhere randomly 2.) The "whistle" sound when you drag the ball really kills the mood for alot of song for me :/.
Anyone knows is it possible to somehow remove the ingame sounds and that anime girl? Thanks

Load up osu, go to options, and turn off combo burst for the "whistle" sound I suppose you can turn off SFX as a whole in the Audio tab of settings It will turn off all hitsounds and stuff though

If the whistle kills the mood of the song, that means the song is poorly mapped, the sound effects should fit with the original music. Turning off sound effects lowers your skillcap, once you do that the game is just about clicking fast, and not about following a rhythm, so naturally you wont be as good at the song as you could be.
The chick doesn't pop out of nowhere randomly, when you get a combo going a sprite will appear at certain intervals, 30x combo, 100x combo, etc. I don't know how you could find that it can "annoy the crap out of me".
Just play the game more and then these minor annoyances start to become less significant

I agree that the whistle hitsound is REALLY annoying though, and some maps use it really badly. http://osu.ppy.sh/s/26512 on insane for example, uses a whistle on every slider, and often has sequences of 6 or so sliders in a row. TWEETWEETWEETWEETWEEetc
On some streamy maps also, mappers will make every 4th note a whistle, and since the whistle is louder than the other hitsounds you get the same problem.

A few tips:
Turn off Combo Fire and Combo Burst in the options. (Combo Burst is the badly drawn character popping out in your face when you hit a big combo)
Having Hit Lighting and Shading Effects on is a matter of personal preference, its like how the pros at Starcraft play on low graphical settings. Turn them off if you want minimal distractions. I keep them on for the awesome effects alot of maps make use of.
Set the frame limiter to Unlimited
Don't use any form of Mouse Acceleration (You probably already know this)
Dont turn off the effect sounds (Hitsounds etc) if you dont like the whistles on certains maps etc etc, rather, delete that map (Because its probably bad), and get better maps
Change the skin to a something a little less distracting, http://www.mediafire.com/?6bzgujijv05146h is a good one
Id say playing at max resolution and fullscreen is optimal when having a widescreen monitor, i'm not too sure about 4:3 though, i used to play windowed mode 1280x800(widescreen) back when i had a 4:3.

You can also make your own cursor by going into the osu folder, taking one of the skin folders in there, copying it, and modifying just the cursor in PS or something that can save transparent PNGs, then picking your skin in the options menu. Personally I made mine black/red with a bright purple dot in the middle, I find it easy to see on almost every background (not like you look at the cursor much anyway heh).

Yeah I don't understand it either, I mean it's personal preference but playing without hitsounds is so much harder than playing with them. I can understand turning them down if the whistles/claps annoy you but to play with them muted is just making things 10x harder on yourself lol

I would think that should help you though, so you'd know if you're hitting too early/late and can adjust your tapping accordingly.

It doesn't cause me problems like it used to (now, I can play close to the same with hitsounds on or off, although my streams are still a bit more accurate with them off), but for a long time, hitsounds used to lure me into "incorrect" rhythms, which would utterly trash me. It still happens to me with streams sometimes.
Hitsounds can be useful when you're to the point where you hit 300s virtually every time and 100s are a rare abomination, but as a beginner, it's much easier to keep them off. Streams are also much easier with them off, IMO, since streams require that you hear the actual music as clearly as possible with no interference. Once you get a bit better, hitsounds can make some quarter-note sections slightly easier, and they're more fun to play with (unless the mapper was an asshole and used whistles).

Interesting point with the streams, I think I will try this out and see how it goes.
EDIT: Nevermind, I just suck. But the hit sounds might not be too important to hear for some people, at least it wasn't for me. But I personally find the hit sounds very satisfying to listen to.
