In terms of speed and overall gameplay efficiency, your totally right; a strong understanding of the game and good fundamentals makes a much larger difference than what hotkey layout you're using.
On the other hand, when talking about ergonomic efficiency, they can make a huge difference, especially if, for 8 hours a day, you sit at a computer typing. Different peripherals and changing up your hotkeys and hand position can mean the difference between no pain and carpal tunnel.
