Well, actually the title says it all.
I am a Plat/Dia Zerg, and I only use 3 control groups: 1 = Army 4= Queens 5= Hatches
I think my game would improve significantly if I would properly use 2-3 too, esp as I like infestor play. Everytime I do it though, I mess my controls up in battle and end up putting everything on 1 again.
Is there a good map, or, which is the best custom map, to practice it on? I'm sure there must be something to train this?

In terms of the normal melee-style maps, there isn't any specific map that will be able to help you get used to control grouping any more than another; however, chances are that there's some custom map out there that helps train you in control groups.
My advice is to just establish a control group setup and just stick with it every single game until it becomes second nature to you. For example, I use:
1 Zerglings/Ground army 2 Infestors OR banelings, depends on my army comp 3 Mutas OR Corruptors 4 Creep Queen 5 Hatches 6-9 Queens
The most important thing about control groups is that you're able to use them faster and more efficiently than you could before (e.g. minimap clicking and boxing)

As I understand it, battles where you use more than one hotkey for your army go badly; this is the symptom of what is wrong. The reason it is going badly could be several things:
1: You know what you want to do, but your fingers dont find the buttons fast enough. Fix this by placing something you use all the time (queens, hatcheries, main army) on the new hotkey instead of where it usually is. Repeated use of the hotkey will accustom your fingers to finding it.
2: You know which buttons you want to push and your fingers find them, but you have to actively think about it in battle and everything seems to be over by the time you're halfway down the list of things you wanted to do. Fix this by repeatedly playing the exact scenario, in your case army+infestor control. Get a friend to help you out, decide what actions you want to do in a battle beforehand and then execute them as fast as you can. Rinse, repeat

nah, I only have 1-2 control groups for my army. I do use 8 control groups for my production/scans/upgrades....

I'm not really sure about how to force this apart from just doing it. If you're ever in a situation in which you're thinking, "Damn if I had another control group I wouldn't have lose those units," then that's probably enough of an impetus to try it.
I guess an easy way to make it happen is to do mutalisk play since you HAVE to have a separate control group for harassing. Once you get used to attacking with your main army and having mutalisks harrass at the same time, you should be decent enough with 2 control groups. Also force control groups for whatever you feel ever needs to be boxed and moved. Even if you go roach/hydra, put them on separate control groups since you usually need to keep hydras in back and keep scooting roaches in the front.
I recommend also adopting a technique I picked up from Titan from Mr.Bitter's 12 weeks with the pros where he assigns units to control groups as soon as he makes them by control clicking the eggs of a specific unit type and shift appending to the appropriate control group. It pains me when I see players lose pieces of their army because they're standing still at rally points and they're busy microing or macroing.
Who knows, this might help keep separate control groups as well since sometimes you dont want your reinforcing units to stream in (let's say you're making more lings to turn into banelings).
Most important of all I think that playing in a way that benefits greatly from using control groups will force you to use them (and appreciate them).

I like having 2-3 control groups.
One group is all my units (all of them) the other is all my combat units. The third is my non-combate/harass units.
The idea is that you want to be able to attack move your army around the map--but then be able to break them apart at will. This removes the annoying 1a2a3a syndrome just to move around the map.

Every single game you play will help you change your hotkeying habits. Just keep playing games with a new hotkey set up. It will feel awkward and stupid at first, but eventually you will learn to adjust. There is no quick ways to change something that you've done hundreds of times before.

I will try to work on this, first the "easy" way with mutalisks... Then maybe more micro intensive harass units, such as banelings, and I think that should do the trick.
