It will take a while but it's easiest to scout with a fast air unit; get many of them and send them all into lines across the map, so you don't miss anything. If he's really that annoying, arrange your air units in an array via minimap to cover the entire map (will take a while, but is probably the most reliable method)

Why doesn't some zerg just get a quick 2nd FE when meeting a protoss FE (@20 supply or so)? Even on really large maps like shakuras, x-position?

Typically the center patches on most maps are far patches whereas the close patches are on the sides. Close mineral patches offer a slightly higher return on minerals than far patches, so people will attempt to mineral stack workers on close patches (2 per patch) to get a mineral boost (to the order of 25-30 minerals) in the early game.
(This is, of course, dependent on the map and the positioning of minerals at each base on the map. Older blizzard maps have fairly pronounced 4 close/4 far patch configurations whereas with later maps and iccup/gsl maps, the distinction is less obvious.)

Many zerg do, but you need to weigh the ability to defend your third from protoss harass, especially when the typical follow-up consists of stargate play. One important factor here is your ability to get good creep spread to your third, because the typical early defense (queens) won't be mobile enough otherwise.

2 other reasons not mentioned yet:
1) Economically it's not even worth it to get it that early. You need like 40 drones before a 3rd base becomes useful.
2) Doing so puts absolutely no pressure on the P. He can get away with 1 cannon and all chronos on nexus, and again, you're actually pulling ahead that much economically relative to the P (if at all).

Is it impossible to spawn close positions on Metal on ladder, or was that just a TSL quirk?

It's a MLG quirk. TSL used MLG versions of blizz ladder maps (such as shakuras and metal).

These can all be tested in a unit tester custom map.
Go out and try it, I don't think anyone knows the answer for these very obscure questions

Nothing beats a good old regular game. Play against a low level computer with one build in mind over and over again. Practicing your mechanics and getting them clean is the best way to do it. Concentrate on constant worker production, not getting supply blocked and expanding safely.

Is scanning to kill an early game observer always worth the scan?

I know that 30 probes per expansion is ideal, but I have this bad habit of building wayyyyy too many probes. I just played a PvP where I had 95 probes in 2 expos with my main mined out. Because of this I have trouble spending all my minerals fast enough and even with me trying to hit every warpgate cycle I still end up with too many minerals and no enough gas. What is the best way to fix this?
