NaYeBo's Terran thread seems to be garnering some good info, so I thought it wouldn't be a bad idea for a similar idea applicable to another race.
Following the format of:
"When you scout your enemy base, and see _____________, this most likely means __________, and you can _______________. "
For example:
"When you scout your enemy (Protoss) base, and see 2 Gateways before Cybernetics Core, this most likely means you will be zealot rushed, and you can make a bunch of lings and one or two spine crawlers."

"When you scout your enemy base, and see 5 techlab rax, this most likely means 5rax reaper expo, and you can get roach speed asap and lings to counter the marauders. " i hate helion openings more than reaper openings

When you scout a forge and cannons walling off the Protoss natural and main, you can expect Void Rays. Get mutas asap and extra queens.

when you scout protoss and see 2 gate into cyber you can make roaches and an extra hatch to defend the imminent zealot into stalker push
when you scout zerg and see the quick pool you can make 2 crawlers and 6-12 lings then drone to get more of an economic lead
when you scout terran and you see the second rax go down before your scout dies you can go heavy ling/baneling into muta into roach/infestor to counter his mmm (/tank) ball
when you scout a none cheese build you should aggresively expand. when your queens start to run high on energy you can make another hatch in close proximity to use their excess energy on.

When you scout your enemy base, and see terran or toss doing whatever, this most likely means you need to drones or lings, and you can 13gas 13pool speed lings research because with fast speed lings you can defend against any push. You can safely expand. You can fast lair. ....................As zerg, I want that map control because I'm zerg. I want that map control from early game to end game.

When you scout: Protoss and see (I usually play 1 base until i counter the first push by protoss): 2 gateway before gas or cybercore: Expect heavy zealots, counter earlier pool with speedlings/roaches 1 gateway 1 cybercore: Expect tech and stalkers, counter with speedlings and +1 melee/carapace no pylons/gateway/forge: expect that your being proxy gatewayed, counter with Immediate lings/spine crawlers 1 pylon 1 forge no gateway: Expect proxy cannon rush, counter with 5-6 drones killing the pylon in your base, early spawning pool, spine crawler near the edge of the creep (not in range of the cannon) 2 Gas 1 gateway 1 cybercore: Expect tech (DTs/HTs/Void Rays/Collusus), counter with early speedling push, 3 queens, fast lair into hydras
Terran and see (I usually fast expand vs. terran): A wall in my face (fast wall, 2 depots, 1 rax): Expect fast tech, seige tanks/banshee, counter with Banelings/speedlings, BUST! lair for overseer OR evolution chamber, spore crawler. followed by tech to mutalisks (Many many mutalisks). 2-3 rax no gas: I expect marine push, counter with speedlings 2 rax (at least 1 tech lab) 1 gas: Expect reapers/marauders, counter with spine crawler, roaches/speedlings 0 rax (or very late rax) 1 gas: Expect proxy reaper, early spawning pool early queen, spine crawler if you can, 3-5 drones where you think he'll jump up from, try to scout the rax with first lings 3-4 rax 1-2 gas: Expect Marine/Marauder push, counter with mass speedlings, tech to mutalisks.

at what food do you usually scout (assuming Ovie is too slow) and does your scout timing depend on opponents race?

When you scout your enemy (Zerg) base, and see an early gas and pool, this most likely means lots of banelings are coming, and you can either get banelings of your own or die trying to get enough roaches to fight it off. EDIT: seriously, I hate ZvZ

I like this thread. I'll try to throw in some stuff that isn't covered yet. Feel free to comment on anything that is not necessarily correct.
vs Zerg No gas before pool finishes (later than 10 pool) = check for/expect an expo More than one drone coming to your base to scout = spine crawler rush. ~3 drones per spine "Vibrating" Spawning Pool right after pop = Ling speed Evo Chamber before 30 = Early speedling upgrades or Anticipatory Muta Defense Fast Infestor Pit + Speedlings = Probably looking at Ultralisks in a few minutes Corruptors when you haven't shown mutas/ spire = Broodlords
Vs Terran -No Barracks = Possible Proxy Barracks/ Bunker Rush. -Second SCV sent to scout = Bunker Rush -On maps like Scrap Station with short flight distances, leave your overlord for just a second in sight of the barracks after it finishes to see if it throws down an add on right away. Techlab likely means reapers. Reactor often means Hellions (Check for the Factory if you can). -If the wall isn't A) 2 layers or B) made of big buildings, go for a bust. Either send an overlord at the wall while banelings are being morphed to see the setup, and if it's prudent. Alternately, send an overlord into his base away from the wall. Time the bust for when the marines are killing the overlord.
Vs Protoss -Early cybernetics core = Feel free to steal their gas. -If ramp has 1 Pylon behind, 1 Gateway flush with doorway, he's planning to throw down another building there. If he isn't chronoboosting the Gateway, feel free to get in his way when he wants to throw the building down. -Gate into Cyber into Forge often means a +1 timing push with zealots, stalkers, and sentries (I could be wrong on this one). I like spine crawlers vs this. -If you scout an early Robo Bay, get an Overseer. If it's in your base, you can keep his Observer out (patrol between bases). If it's near his base, you can contaminate either the robo bay or the robotics facility when he goes for Extended Thermal Lance. (good for buying time until spire finishes)
