i dont like using action bars to track cooldowns so i hid them and made weakauras bars for all of my cooldowns. where things are overlapping it's because they're two things i cant have at the same time due to talents or spec. i moved dark transformation down near my unit frame to keep track of it more easily, as well as blood tap (i use when tanking). i have it set up so sounds are played when some of my more important cooldowns are ready (trinket, gargoyle, empower rune weapon). playing a sound when dark transformation has 12-15 seconds left, as well as having a bar or icon pop up, could be pretty useful for you. i have the dark transformation bar up for the whole 30 seconds just so it's perfectly clear to me when my pet has his interrupt available (leap is also an interrupt when he's transformed). for soul reaper i just look at my enemy unit frame debuffs but you could set up a weakaura for soul reaper too
