What about Secret of Mana? That's multiplayer. So is Final Fantasy 6.
Mario Kart is a good choice but it gets ugly playing splitscreen. Smash TV is fun in multiplayer.
Sonic 2 is pretty fun in multiplayer mainly because the second person can really go for broke. If the first player is really good though you just get left behind a lot (and some levels don't even feature Tails). Cyborg Justice is another fun beat-em-up with some neat twists for Genesis.
Firstly i'm impressed you didn't break up playing Street of Rage together. The game's designed to cause arguments. Why not go for the sequels and/or Golden Axe/2 and the various Battletoads?

if you're into action rpgs, try secret of mana/evermore and seiken densetsu3 they're awesome rpgs in their own right and even more fun on multiplayer, kirby superstars had an awesome co-op mode as well
Lost Vikings 1-2 would be my suggestion because they're relatively peaceful so not too much dying/frustraction. They're really nice puzzle games.
Secret of Mana, Secret of Evermore, and Seiken Densetsu 3 are also good picks that will last a long time. They're all pretty decent co-op action RPGs, though I'd say SD3 is the best. Plus for SD3, you can pick your 3 party members from the 6 available, so if there's a character type that she wants to play, there's a somewhat decent selection.
Since you are willing to play a game in a more hardcore genre (props yo, not many could make it with gf), the best co-op beat-em-up or run-and-gun game on those systems is probably Gunstar Heroes. + Show Spoiler +
Of course, Contra, Super Contra, and the like are more than fine games too.
Unfortunately, those systems don't have too many groundbreaking or really fun plane shooters with co-op mode that I can think of. A lot of the better games in the genre happened later IMO. But some examples are Dragon Saber and Aero Fighters.
Are you just limited to NES, SNES, and Genesis, or could you emulate arcade games? With arcade games you're never going to run out of stuff. Playstation and Saturn open up a whole realm of games as well.
Secret of Mana is such a sick game--definitely do that! It can be a bit annoying though because one person is basically going to be the leader and the other person will just follow since there isn't a split screen or anything like that, but battles and fortresses and what not will be so much fun!
WHAT!??! HOW HAS NOBODY MENTIONED KIRBY SUPERSTAR YET? [for SNES]
1) Kirby Superstar Hours upon hours of fun. The better player can play as kirby, and you can eat enemies and spawn a helper [p2], who can play as a meat shield/support and get re spawned when you die as long as kirby is alive. PLUS when you eat food and you walk next to each other you share food which looks like you're kissing @_@. The boss fights are fun, the music is hapeehapee, and theres a huge diversity in ability choice. A must for any couple games! Plus theres a low penalty cost for p2 to die, but it seems your gf is kicking ass so might not apply here =p.
Besides the six different length and theme campaign modes, there are also mini games to be unlocked, as well as a boss only arena mode. So theres a lot of replay value as well. If you're OCD you can also try to get 100% on the treasure collecting game, but thats quite tricky without a walk-through.

The modded xbox can run arcade emulators, so you can get the arcade versions of those beatemups like Knights of the Round. The best ones IMO are Capcom's Dungeons and Dragons games.
If you want to play coop on xbox games, try Dungeons and Dragons: Heroes. That is a lot of fun.
:o old school co-op games, never thought of that, me and my gf have pretty much done all the good co-op games on 360 (gears, halo etc.), now we've kinda just degenerated into playing 1p games and taking turns (which can get frustrating, lol), this thread will help me a lot :D.
Pretty much all good games that come to mind for oldschool games have been mentioned i think, ill 2nd bomber man as i remember that being hella fun, haven't played it in like 10 years tho so could just be nostalgia xD.
also - not to derail the thread but i guess its kinda on topic, does anyone have any good suggestions for co-op wii games? (pref proper, storyline, games. Dont say "Wii bowling is fun! :D" >< )

Easy solution: Let her do the walking around. Or take turns.
And yes, kirby superstar is win. Pretty much all kirby games are.
wiz n' liz on genesis was one of my favourite games to play with my brother/sis as a kid :D it's a bit hard to describe so i'll just link a vid.. your gf will probably find the cutesy/cartoony side of it rather appealing
obviously one of the easiest levels.. can get rather taxing later on and there are tons of mini games and some really fun/interesting boss fights too
I strongly recommend Secret Of Mana 2. There is no better Action RPG with 2 (or even 3 Player) mode. In the first hour you will have to play alone, but when you meet the second main char you can play together. The Fights are just awesome, full of Action but combined with a very good story, great bosses and so on. It's the SNES game I spent most time with. When you prefer Jump&Runs Donkey Kong Country 2 could be another option for you.

Actually several people have mentioned them already.
I played the Golden Axe 3D remake on the Genesis Collection for PS2 and it lots a lot of the appeal because the models were so low-poly and the graphics were horrible.
Virtua Cops (I think that's what it's called) is a great simple 2P co-op stage-based shooter game to play with a girlfriend. One persons is on keyboard, other on mouse (mouse is 10x easier to play with).
Sunset Riders is great, as others have mentioned. Another great game, which also has a western theme, is Wild Guns - great arcade-style 2-player game, used to play it with a friend a lot.
Tetris Attack -aka Pokemon Puzzle League aka Panel De Pon. Despite the name, it is nothing like Tetris. It's really Panel De Pon, but Nintendo change the graphics around to gain mass appeal. It's more forgiving on mistakes than other puzzle games while allowing some massive fullscreen combos.
J.League Excite Stage '96 -The game is japanese, but it's a great soccer game. 4 players total. I just played it today.
