It doesn't apply equally to all players, it's a rule of thumb based on the mean deviation observed across the player pool. Even though the deviations change a bit, it doesn't impact this number very much, so for all practical purposes you can consider it to be constant.

- Yes, you can see the parameters in the github repository (rating numbers are scaled by 1000 and shifted by 1, so 1000 displayed on the site is 0 in the code).
- We lowered initial uncertainty a bit. (Same file, you can read the history. Note that this might include changes that were never pushed to the server.)
- Could also be related to #2. It's the only thing I can think of.
