I've never seen an inject timing score so high. 92% is indeed the theoretical maximum unless you get more than one queen per hatch.

I go for a standard rax and a proxy rax on cloud kingdom.
He doesn't scout, goes 1 rax into gaz, when i put down my second rax in proxy he cancels gaz immediatly. (2.16)
He then proceeds to throw down a blind bunker. (2.56)
I scout it, pull all scv's as i am all in, he then (with no scouting information AT ALL) closes the wall and pulls scv's to repair bunker. (4.20)
The whole game he only saw my scv come scout from my proxy rax (and i sent it from an angle he couldn't even know i was proxying).

Very very likely this guy is maphacking
He 6 pools in zvz, sends his overlord to the side of the map for no reason.
I'm 10 pooling him, 8 lings into expo.
He then never attacks with the 6 pool, builds 3 drones, then spine and gas, builds lings exactly in time to meet my incoming lings.
He never moves the camera except for placing the ovi at the side.
He 6 pools or does funky builds in all his previous games.

I've played Mekar a fair bit in both tournaments and on ladder and while we don't talk that much I've never had any reason to suspect any kind of cheating.

First link is a very suspicious opening (in base forge and cannon in mineral line vs early pool), but given you're accusing him of map hacking and it's clear you've played before it's impossible to say whether that was metagaming or not. 8:30 slight move out to attack lings is supicious, though not conclusive. Further in game accusations appear without merit. Impossible to confirm with just this replay.
Second link contains accusation of hard counter to muta by blind phoenix. Blind muta from you was in response to AMG already committing to 2 stargate phoenix, with the second stargate almost finished building and some phoenix already out when your spire is even placed. Commits to air play before you've even started your lair and confirms it before you've even taken the gas that'd suggest it. There's no way this was a hack unless he can see in to the future.
No suspicious map peaks on either and only the one odd interception. Certainly the first link contains some strange play but it's not conclusive, but there's absolutely no evidence for the second.

Just played a suspicious game where protoss did 0 scouting and never moved his observer away from his army. He had zero vision of the map up until he made DTs and moved them to my base in a FOUR player map. He also blindly tried to intercept my push in the fog of war without any vision.

Oh there's a lot of things that indicate he's a maphacker:
a. that inbase pylon into nexus first and gateway soon after is so dangerous unless you go for hatch first.
b. goes for very unsafe wall that isn't even a wall on both ends just to hide the cybercore, with no scouting at all.
c. intercepts the overlord, moves the stalker at the exact second the ovi begins to move out of his vision and then when he realizes it's going to the main moves the stalker and sentry to main, again, without seeing the ovi moving at all.
d. walks to your mid 4th base location with the immo all in, your army is there to meet him in a huge arc,, goes back without seeing it to the bottom 4th.
e. blaming you for MH and acting like a moron, quite a big tell.
