Looks like it's possible to have 2 deathrattles. At least my loot hoarder had 2 when I gave it Soul of the forest.

so, what would happen if you tried to shadowform when already in stage 2 shadowform? I would assume nothing happens, or the hero-power is replaced by a new "deal 3 damage" hero power, but in theory the game could be programmed to simply add +1 damage to the shadowform in which case it would become a "deal 4 damage" hero power, but does anyone actually know what happens?

The only scenario where you can double hero power is when you Lesser Heal and then use your first Shadowform. It will let you mind spike or mind shatter (if you were to play 2 Shadowforms in one turn) off the bat.

I run a troll priest deck in arena with 3 Shadowforms, so I had the opportunity to test it
