What leauge (and/or rank) are you in and are hoping to become? Top 25 Diamond hoping to hit Masters
What do you hope to get out of a lesson? I hope to improve my mechanics and just my general knowledge of each matchup also hopefully learn how to not die to toss 2 base all-ins.
About how many hours a week do you dedicate to Sc2? All I do is eat, sleep, and play Sc2 right now.
What are your goals in sc2? To hit Masters and then hopefully work my way up to GM.
What are your weakness in your gameplay? My main weakness is scouting and defending Protoss all-ins, besides that I tend to have problems making late game decisions in terms of engaging and army positioning.
What are your strengths in your gameplay? I feel that my ZvT and ZvZ gameplay is pretty solid in general (whenever I do lose I can easily identify my mistakes and improve from them) It's only ZvP that's a real problem for me.

What league (and/or rank) are you in and are hoping to become? Plat atm, hoping to become much better and improve. Masters and maybe beyond.
What do you hope to get out of a lesson? Improve my macro, mechanics and decision making. Hoping to improve my pvp
About how many hours a week do you dedicate to Sc2? 7-10
What are your goals in sc2? Not just increasing my ladder rank (though thats nice too  but becoming better at starcraft :D
What are your weakness in your gameplay? Macro, poor decision making, my pvp is pretty bad right now as I am trying to learn macro builds.
What are your strengths in your gameplay? I guess my micro is pretty good, not like that really matters in plat anyway though

With some proper training and sincere effort, anybody should be able to make top diamond/low master range. Even I did it...

What do you hope to get out of a lesson?
The main thing I'm looking for, is to have fun! :D The lessons have to be fun, cause that way I'll learn more.
About how many hours a week do you dedicate to Sc2?
Hmm, I really don't play that much, yet! But I watch a lot of videos every time I have lunch and stuff like that.
What are your goals in sc2?
My biggest goal is to enjoy doing it, but also to be able to see improvement, and maybe one day beat my friend.
What are your weakness in your gameplay?
I think that my biggest weakness is to multitask and to macro properly.
What are your strengths in your gameplay?
My micro is quite good, I just tend to focus too much on it, and forget about macro.

I want to improve in general, but would also like techniques to help myself generally improve.

I will probably dedicate 7 or so hours a week to starcraft.

Um, probably everything but specifically I have a tough time with multitasking and not forgetting things.

Well maybe micro. But again not really great in anything.

What do you hope to get out of a lesson?
Knowledge about what I am doing wrong and what I need to practice. Practicing the wrong things isn't beneficial :p
About how many hours a week do you dedicate to Sc2?
Depends. Sometimes I play 5-10 games a day 7 days a week sometimes I slack off.
What are your goals in sc2?
To become a professional player or manage a team or tournament.
What are your weakness in your gameplay?
If I knew I wouldn't need coaching TT. I guess it would be my awful micro and my inability/hatred towards using the infestor.
What are your strengths in your gameplay?
I have no idea. When I'm in the game I kind of just play and sometimes my units die and sometimes his units die and I don't know why TT.

What are your goals in sc2? To become an amazing SC2 player and eventualy be able to enter and hopefully win tournaments
