Im sure you know this already but,
Take a break if your hand hurts, if it hurts a lot or seems to last for a bit then see your local doctor.

I used to play with my entire forearm on the desk and would lift my wrist to move the mouse around the screen. (Was part of TWL when I was in Highschool.)
If your desk is big enough I would suggest doing that. If you can't do that you could just try to lift the mouse instead of draging it accross the pad/desk. OR you could wrap some tape around your wrist so that it isn't directly getting friction from the desk.

Either put a book or something under your elbow to elevate your wrist so that your hand's not in contact with the desk, or move your mouse closer to the edge of your desk and mouse there. You'll want to sort of hover your wrist if possible, but that shouldn't be too hard if you're near the edge.
(At least that's what I do.)

I did take a break, i'm on my 6st day ><

I use a gel mousepad. Your hand lies on a soft underground then. Maybe thats not good for 200 APM, but for my under 100 it works quite well.

(At least that's what I do.) The edge of the table is interesting!

try moving your hand back towards the edge of the table.
I'm not sure if bottom right bone you mean the far right bone in your hand or your wrist. Either way you're probably holding your hand at an awkward angle that could be fixed by literally just moving your hand closer or a bit further away. Also, remember to stretch your hand and wrist and take breaks if you need to between games, if only specifically for that.

Does anybody know if i should be moving my mouse with my shoulders (or elbows)? vs wrist?
|
You should technically be using your forearm, though I know it might feel a little sluggish if you're used to using your wrist to "flick' the mouse; using your wrist too much can cause wrist and finger pain, though.
Also, make sure your desk is set up ergonomically. If you hover your forearm/elbow and your desk is too high and/or your seat is too low, you'll end up putting a lot of stress on your right shoulder, so watch out for that.

I actually use a 2nd chairs arm rest while I play. I've just gotten used to it over the years I guess but its a really strange way to play now that I think about it o.o either way its mostly my wrist that supports my hand.
