For some unknown reason, I used to have a bug where pausing the game would make me unable to chat. As you can see this might be a problem, and even though it fixed itself for me if someone pauses without saying anything I just assume they have this bug and wait for a bit. Unless its an obvious BM pause.

having someone waiting in-game on pause in sc2 is not unlike having a prossy waiting for you while you deal with other stuff, and then refusing the pay the prossy for your break time.

Pub rts 1v1 ladder, you do the math.

No. See my previous post; basically 1 minute of my opponents time is worth less to me than the points. I never pause for more than 1 minute, and I never pause unless I absolutely have to. If my opponent can't wait politely for one fucking minute when we are engaged in what is literally a one on one battle of wits and will...fuck that guy.

Ask "go?" or something similar before you unpause, I hate people who unpause without asking , it is worse than denying a pause at all imo.

If I need to pause I usually pause then immiedietly write something like "sec, phone".
if someone pauses I usually write the following:
"why pause?"
"I'll give you 1 minute to answer and then I'll unpause"
and then if they have a valid reason to pause, I'll wait an appropriate amount of time and if they are not back yet, I'll say
"go?"
if I don't get an answer I'll say
"I'll give you three minutes to answer before I unpause"
if he answers, repeat until I get too impatient.
p.s. I pause before writing because I'm not a very fast typer, thus I pause to get the time I need to type.

EDIT: TBH Blizzard really should do something about it. If you need to fix your hotkeys or a key got stuck on your keyboard you should be able to pause and quickly fix it without any problem. Since you have 3 pauses, if Blizzard could make it so that once a player paused the game, no other player could unpause for 10-15 seconds that would help a lot.

These games where people auto-unpause for no reason or offensive gg are the only ones I can get pretty BM myself and will lift all my buildings to send them across the map (if I loose). It once took them over 20 minutes to kill them all on tempest. I had time to go walk the dog and come back to see a pretty funny chat log. =)

First of all - you need to give a reason if you want a pause from me 
Random pauses will always lead to an unpause.
For everything else you will get enough time to tell the person on the phone to call back, or to look who's at the door (signing for the mailman). So ~2mins.
If it's something important - just leave.
It's only a game (funny how that always works in both ways - not only the "it's only a game, you can wait 10min" way).
Oh btw - if the game is <5min old and your reason is "bio" - no pause

The only legit reason to pause is for technical problems, but the game has been out long enough that this really shouldn't be an issue.

You are correct, it is there right to unpause if they want. Just expect to be called out for the dick move that it is.
Metaphorically speaking it is like when an old lady drops her groceries and she is struggling to pick them up. You dont have to help her but people are going to think you are prick for not offering to.

In this case, I don't think it's the win or the points that matters. It's the fact that you can make someone so very upset with so little effort. That is the true reward.

actually, to amend your metaphor.
it would be entering a shop, an old lady drops her groceries as you walk by, then she presses a "you'll help me pick up my groceries"-button and you have no choice but to either help her or be labelled a dick.

pausing is easy. Just pause say second if you are nice even explain things. But its inappropriate to pause in any battle, you will have to wait till its over before pausing (find a good point to pull back to pause for example).
And if they unpause at any moment when you are gone, send the replay as a report to blizzard. If they unpause asap just chat with em, they will flame you anyway and you can report them as well.
The biggest problem is that people that you cannot really disable this, otherwise the people that unpause now will pause when they are loosing. So no solution to this is possible other then remove pause complelty ^^.
Only way if such people upset you is to report every single one of em ^^. Atleast you supported blizzard with it because kiddos like this will buy another copy asap.
