I've also updated the warnings and banned game counts, removing all entries for "removed from the ban list" older than 6 months. This brings the list in compliance with the new rule I put that only uses your banned game count from the past 6 months, rather than from all time.

Also, all "removed from the ban list" entries for the last 6 months have been updated with dates, so that it's easy to keep this up to date in the future. Also, date format has been changed from MM/DD/YYYY to Monthname DD YYYY to prevent confusion between Americans and others about what the date means.

This new rule I think has been well-received so I feel comfortable making this change. Let me know if you see an errors in the list now.
