As far as I know, lots of orgs do take good care of their players in a sense that they have them do physical training (individually and as a team), talk to nutritionists, get help from physiotherapists etc. However, the average gamer doesn't to the extend we should, especially when sitting a lot.
This seems also to be the case for semi and casual teams, mostly because they lack the resources. In this episode, Nina points out how smaller teams/orgs can make it even when money's tight.

Not a specific fitness routine, but what really helped me was (not talking about general healthy stuff like sleep, food, and physical exercise):
- having a height-adjustable table and actually using it!
- every time I go back to sitting (from standing), I do 3-5 mins. of easy stretching
- increasing my eye health by a) taking 3-5 min. every 90 min. (I work in 90 min. cycles - look up the ultradian cycle if you want), and b) take supplements.
