What kind of re-arranging and/or posture changes do I need to make to stop this from happenening as I've seen that decreased blood flow in the wrist/hand can favor carpal tunnel. Simply not resting my hand is not an option because I've played like this for many months and I don't think I would be able to make such a dramatic switch.

Alternatively: Try having elevation of your wrist and make sure you are sitting (Leaning a little backwards) upright, Also see where your table is hitting your arm, is it at the end and cutting at your wrist? Is it laying on the table?
