When vets are banned, they're usually banned without the mod asking around to other mods. However, in a way, no perm on an account with lots of posts is ever done unilaterally. Some users accrue a whole bunch of mod notes (warn/bans) and it just takes one mod to get exasperated and finally pull the trigger.
