After a lengthy discussion amongst the moderation team and admins we've come to the conclusion that Roffles' posting in recent history is not the type of posting we want on TL. As such we have permanently banned him, independent of the legitimacy of the original ban. Roffles abused his former staff status to create an annoying grey area for moderation -- typically people in his position are granted leniency when posting on the forums and as such can get away with things normal users are punished for. Roffles abused that privilege on multiple occasions resulting in temp bans that would have otherwise been perms for regular users. We feel that the repeated mod actions and his unwillingness to change his behavior justifies his removal.
We appreciate that there are other offenders who have been posting in a similar manner, probably sponging off of the "legitimacy" of Roffles shitposting and avoiding mod actions. This is going to stop, and we're going to contact the major offenders of this letting them know that they're on thin ice. If they do not change their behavior then our hand will be forced.
EDIT: discussion can continue about this ban, no need to discuss the other.

do mods/staff have like a special page, where they can look at an organised list of like all the bans that have happened over some period, and the links to the banned/warned page?

Yes, we have exactly that 
Each user with mod actions gets his own mod report thread, and each warning/ban shows up as a post in that thread for us with links to the post, reason, type of mod action, etc. Makes it so we can judge users quickly by being able to see their hwole history when doing moderation.

I remember his name from several occasions, but sadly without any details, except that he has been here for quite a while, was a staff member and has a high postcount.

Mods can make their own comments on them too but 1 page on TL = 20 posts so the 41st post in your mod notes would start page 3
