Any idea why is Lina Middle so low on the list given that pros have recently run Lina mid showing it off to be a 'thing'?

thanks to the core items that chases foes (number disagreement)
the likelihood of Chain Lightning occuring are quite high, allowing you to farm fast and tear down teams in fights impressively quickly. (misspelled "occurring" and a number disagreement between "lightning" and "are")

w.r.t Nyx mid guide, if you are going to say "don't get this item if you can't build it by 20 minutes" for Dagon you should provide a Situational alternative.

I'm glad to see you are a staff member (can you change the title to 70 instead of 60)
I'm a bit busy this weekend with the launch of issue 3 soon; but I'm prepping some format posts so I can immediately start drafting changes needed to be made.
I will be leaving for Tallahassee on the 7th; so this is an ideal time for a patch change for me.

Starting items are going to change for a lot of heroes with the change to magic wand (now uses a circlet) and addition of mangoes and stout shield cost reduction

Rank 1 Wall is better, but the reason you delayed wall wasn't because rank 1 was bad, it was because Wall isn't really that good until Vacuum is high enough rank.

Your Bear actually needs to do enough damage for attacking out of range to be worth it and you dying first is like a complete nonfactor early-midgame with the sheer amount of free stats from True Form/Synergy. It's extension at earliest, really because you're already going to have 1-2 major items by the time you'd consider it, and a lot of the time by the time you'd want it is when the Bear's problem is that it dies too fast more than anything else.

Octarine Core is a must have on pretty much any spell based carry.
OD, Pugna, DP, Necro, Enchant, Silencer, Leshrac, Lina, QoP, Earth Spirit, Silencer, Timbersaw, Phoenix, Zeus, and core Sky/Earthshaker at least. If it interacts with Ward damage then also core on Witch Doctor and Rhasta.
Situational on anyone with a big ultimate that already buys refresher. Not sure if Refresher is better though.

I wouldn't call it "must-have" more than "will probably get by 6 items".
A lot of those heroes want to be ending the game before they could feasibly get the item to begin with. Even then it's questionable on some of them (e.g. Core shaker typically develops a physical damage lategame rather than a spell-focused lategame).
