Yes maximum cdr is 40%. With 30%+ you cast a ball at 0s, at ~2.8s, 5.6s, reset the lifespan of ball 1 by throwing it with W, cast a ball at 8.4s and you have a fraction of a second to ulti while you have 4 balls out for a maximum of 7 balls. I'm just going by the numbers so you might need a bit more than 30% cdr to do it effectively in practice. DFG/Grail+blue buff or some other combination of cdr seems mandatory.
Getting 3 balls out at once isn't too hard, one at 0s, 4s, throw ball, summon ball at 8s and you have 2s to ulti with 3 balls out. Rushing codex for a cheesy ~level 7-8 kill with your ulti could work.

W doesn't reset sphere lifetime. In fact, it doesn't even extend sphere lifetime unless you grab one after it starts glowing blue to indicate it's dying, and even then it only extends its life for as long as you hold onto it.
The upshot of all this is that having more than three spheres is completely impractical, even at maximum CDR. There's the barest, most miniscule fraction of a second where you can have four, but only if you have the kind of reaction times that allow one to grab TF's first card intentionally, and only if battle conditions permit you to hold onto your slow for that long.

Ok, so I've been trying to learn how to play Hecarim, and he's a lot of fun but I have a quick question as to which summoner to take on him (besides Smite, obviously). Right now I'm taking Ghost because pretty much all the guides recommend it but it just doesn't seem so great. You already have a move faster skill built in and your passive already grants you immunity from object collision. I'm thinking Exhaust is better to help with ganks since you don't have much CC, or else Flash because Flash is always good.

I'd say very few junglers have stronger ganks with Exhaust than with Flash (Shyvana is one of them, can't think of any others though). The main point of Exhaust is actually winning a fight once it starts. I'd say take Flash for better ganks or Exhaust for fighting strenght, depends on which one you prefer.

I like having Flash simply for going through walls to chase or as insurance for those "oh I really fucked up this time" moments.

Oh wtf, I never thought I'd say it but evelynn might need to be toned down a bit. Ever since people realized that AP Eve is far better than AD Eve and started building her as a dfg first burst mage, she's been seeing more and more play. She's sitting at ~55% winrate in gold and 58% winrate in platinum.
