Honestly I think you would go crazy. The pain isn't the bad part, the problem is you're always thinking about when the next kick will come, and eventually it will drive you to paranoia, isolating everyone you care about. It's like chinese water torture. If the kicks were scheduled, it would be an eeeeeasy yes.

Surely there must be some medical way to numb your balls to prevent the kick hurting. Surely. In the end, I'd have to do it, even if I decided I'd take 499 million, donate it to the Against Malaria Foundation, saving 24,500 lives approximately according to GiveWell, and then use the remaining million to immediately get myself cryonicised. If they can revive you from a frozen state, they can numb your balls with near 100% probability. There is no game-theoretic way to decline this deal. None.
