First, I would recommend budgeting 400-500 for the electronics (amp and dac), and 200-300 for the headphones.
From my experience with closed phones, I think the Sennheiser HD25 sounds very good for the price, but is very uncomfortable (they pinch the ear tightly). The audio-technica esw9 is more comfortable but doesn't isolate as much (but it doesn't make much difference unless you are using it in public transport). I would recommend either phone over any universal in ear monitor, if you do not need the portability and extreme isolation that IEMs offer. I also wouldn't bother with anything made by Bose or Monster.
For amp + dac, I would consider an integrated unit such as Headamp Pico http://www.headamp.com/pico/pico_dac_amp/index.htm or Centrance Dacport http://www.centrance.com/products/dacport/

Sennheiser HD555s. As long as you aren't in a loud area these are probably some of the best headphones for their price and can be worn forever without bother.

The wire on the left cup popped out a few weeks back, the headphones themselves are fine but I have to pop the wire back in a lot. So annoying.

IIRC the AKG K242 HD is the same design/transducers as the AKG K240s, which sells for $100. Those are different than the vintage AKG K240 models, so they shouldn't be costing anywhere near $300.
If you're just looking for an upgrade over onboard sound that can drive those headphones loud enough, the Asus DG sound card is a cheap option with an integrated headphone amp of reasonable power. (Obviously you're not going to get the best amp ever for $30, but it should be okay.)

Why don't you try IEM's as they shouldn't fall out of your ear unless the tips are really small or something They provide good isolation too generally
Clip on is also another choice~

I'm using AKG 701s for over a year now. Nice things, horrendous headband (your head will adjust to it eventually :E).

100 is minimum to get out of the cutesy headphone range where you aren't paying for quality. Under 100 you pay for the look and marketing or the look of the phones (i.e. skullcandy). If they are good quality headphones you can buy replacement parts for them and that IMO is key. IF they sell replacement buds or earpads then they expect the headphones to last longer than the accessories. Also, most good quality brands/manufacturers offer minimum 2 year warranties on all prducts. I like shure for 2 year standard warranty and very high quality customer service. You deal with one real person the whole time and not reference numbers and call centres.

I used to have a pair when I spun records, I can't imagine using them for daily listening though, they're an uncomfortable fit on me when wearing them normally but they're fine when I only need to listen to them through one ear.

Haha w00t! That's exactly what I have and I love it. The earphones are comfortable and have pretty good sounds IMHO and the mic is great. I use it for all my Skype calls and casting and it works perfectly.
