Well, tbh, I am almost certain that he had the idea of his dad being a shanigami and his mom being a quincy for a while now. It's not a hard idea to come by. But it's hard to say if he was sure this is the path he wanted the story to go 5 years ago.

You make no sense whatsoever. Ichigo couldn't do shit before Rukia came, or before he got trained. There's no reason to believe that Karin and Yuzu couldn't be strong if they got trained.

I think one thing that the manga has not explained well is how come Ichigo was the reason Chad and Orihime got powers in the first place if Ichigo's power was basically a fake Zanpakuto or a partial release only? Would it be related to him eventually being the Quincy King/chosen one? Because Quincy's don't normally go around just giving powers to normals around so maybe that's going to be tied in with Bach somehow as Bach is the father quincy.
Or maybe we're going to have a situation where Ichigo is the chosen Bach vessel for him to no longer have a time limit? It would be hard to imagine that bach is actually alive and more of like he's a spirit now with his Quincy powers intact. So maybe he wants to rule over all planes properly and needs an appropriate vessel to house his body after he's shifted the balance in his favor?
