TBH, the difference between D+ and C- is only things like slightly better timings and doing a bunch of little things right that you miss at D+. I'd bet a C feels the same about the difference between a C and a C-..... I know someone in the B ranks that described the same type of differences between himself and a C+ player as well, so I'd guess it holds true everywhere on the ranking (until the B+/A- bracket, then I dunno, because that is a seriously tough line to cross).....
I hit C- pretty comfortably in my 3rd season (not a very active player) with just a bit over 100 apm on average, and about 90 EAPM..... And I'd still consider myself pretty terrible - poor micro, poor macro, and poor timings.
