All TSL Top 60 players must fill out the following form:
If you do not submit both this questionnaire fully filled out and your replay pack by 23:59 KST on December 27th, you will not qualify! Fill it out in advance so we can work with you if you are missing some information
- TSL ID
- Contact Email
- MSN / AIM / ICQ contact (fill in as many as possible)
- First Name / Last Name
- Home Address
- Phone # (both home and cell if possible)
- Full Username ID (without clan tag / team), this is what we call you during broadcasts
- Team / Clan, and your ID with team / clan tag(s) attached
- Race (list matchups, if specific)
- Pictures (HQ and as many angles as possible)
Email the form to PokerStrategyTSL@gmail.com
For #10 - Pictures, you can email this separately later, after you take pictures. These pictures will only be used for TSL broadcasts and advertising.
Zip/Rar your replays from the TSL Ladder into a pack. If you have too many replays for one pack, break it into several parts.
Email the Replay Pack to PokerStrategyTSL@gmail.com
Alternatively, you can upload the replays and email us the link to download.
Please do these tasks as soon as possible. Thank you for participating.
