There would be repeat posts, people talking/posting past one another, probably passive-aggressive bullshit once a poster realizes they were ignored, and on top of all of that - if everyone simply ignores a shit user instead of reporting them, then that makes the job of moderators harder and the amount of shit posts that slip through the cracks greater. No board with an ignore function I've used has run into the issues you mentioned. I can see why they could occur from a hypothetical point of view, but in reality, people repeating the same thing someone had already said is a norm on TL as it is. And if someone can't be civil about being ignored, isn't that a sign they should be banned or ignored anyway?
On a side note, I think this has strived too far into off-topic and will likely be advised to be buried in website feedback.

To be honest, "ignore" button or not, threads are chaotic anyway. The same points are repeated endlessly without including any new argument that might have come in between (that are themselves often repeated endlessly, you should get the idea), people don't care, rinse, repeat. The last three pages of this thread is a good example of this.

It varies. It is intended to mean the report warranted mods do something, such as move a thread, discuss an issue with a user informally without using the warning system, combine two threads together, or something else.
