2) All in one place. Title of the report varies depending on the forum / reported poster, but they all end up in the same forum.

3) There is no set number of successful reports that gains staff's attention when someone is looking to add a banling. Composing good posts, being able to maintain a calm demeanor while participating in debates, and just generally being a high quality poster and contributor goes a lot farther towards getting noticed.
