Xartz, nice probe split in the beginning. In roughly 1 ingame frame (aprox. 1/64 of an ingame second) you selected all your probes, clicked on two mineral patches, deselected 4 units, and send the remaining units to another mineral field. I'm wondering who is doing strange stuff here..

If its just the beginning, or only happens once, it's more likely a lag spike. I'll check the rest of the replay for anything suspicious.

yes, this is a common issue that korona has pointed out earlier. lag spikes can produce this. it is especially common at the start of a game.

Description: I've noticed some obvious known hacker mistakes while I was playing him. He found my pylons as soon as I've placed them and destroy them. Also, I saw the replay, he has never scouted and have no vision on me the entire game. Lastly, he has a "Camera Lock" on so that we can't see where he's looking at. It's way too obvious.
I've played him three times and won once because he's really bad.

Easy now. If he did this I agree that it would be stupid. But as pointed out earlier this can be caused by lag. So no need to point fingers right away.

Check from 10:45-11:20, it seems like he is switching on his blinkhack at the start of the fight (stalkers dont seem to be selected) and the he swtiches his hack off and blinks manually.

Analysed replay only cause I have in friendlist one Makaveli (Russian EU GM player) and he is protoss too. But thats not the same account (that GM is inactive for 33 weeks). And running forward - they play different looks like. That GM spammed a lot with APM 250, but who knows.. Anyway

1:50 - Makaveli builds gateway, then after a second cancels it throws forge. Thats right after HydrOPoneY placed his forge earlier. (having production hack helps, lol). And Makaveli didnt leave his base, didnt scout. An attorney could say that maybe he played you earlier and in all games you canoned him, but he didnt scout, and even didnt try to canon HydrOPoneY. (like wtf? why u built forge and dont even try to canon?). Thats not how guys play in Masters.
2:40 - Looks in HydrOPoneY's base, but his probe starting moveing out in that direction. Maybe he looked there and clicked on minimap somewhere near (i still didnt find the path of that probe).
17:05 - Cancels 3rd Nexus (was almost done) seeing nothing because whole game he didnt scout (except that 2:40 probe scout after late blind forge ,lol). And thats even having blink stalkers in bigger numbers than HydrOPoneY, he didnt even took watchtowers, which is kinda not ok in masters. So he cancels 3rd Nexus, seeing nothing but thats cause HydrOPoneY started moving out in for 2-base atack and Makaveli could see it only on minimap hack.
For me its absolutely clear that Makaveli hacks, and he is realy bad... like realy bad.

Nothing realy suspicious. Just a regular "clean" game. You spoiled to him that you know he hacks and he started to play like he dont.
A tip. If you want to gather more evidence - dont tell if you suspect him maphacking. But in case to laugh only (cause blizzard almost dont give a shit) - ye, its fine.
Having only this replay I'd never accused someone of MH. And bad play alone dont tell anything - he could be listening to music, being under beer, temporarily gave other guy this account, etc... But the 1st replay with blind forge and nexus cancel (sick!) and no scout whole game having 10 blink stalkers (cause he doesnt need to scout, he is just too good, lol) is enough.
ADD.
And he will be in GM very soon if he'll play often with that win-lose stats 37-14. There's not many masters with that stats.
And thats sad...

That's a good catch. I'm like 99.9% sure that means he is hacking. The right click through fog means he selected your building on attack move with a unit, which you could only do with with hacks.

The analysis is correct. You cannot click or attack a structure you do not have prior vision of. This single line is indicative of hacking. Unfortunately, SC2gears times (in seconds) are all wrong, but looking at the replay, the attack in question is at 5:56. Up until that point, the zerg has absolutely no vision of the terran base or tech path, whatsoever.
