Ramsay stabs Roose with this tiny ass knife in front of a completely trustworthy witness and you're there in your full combat gear and Roose's son is a newborn who will need at least 18 years of stewardship. Shout at the maester to get guards, draw your sword and attack Ramsay. Suddenly Karstarks are the house on top for a generation while you wait for Roose's son to grow up. Ramsay definitely has men who are loyal only to him. There would be more than a second assassination. Were they in the room? An absent sword is as good as no sword at all. He can fight off Karstark or escape. If the maester's off to get help, who's guarding the door?
Also I think the maester wouldn't think too rationally given how Ramsay deals with people who fall out of his favor.

Ramsay is just a mad guy with bastard syndrom, the only question will he die in a suffer close to a midseason, or till the last episode. It's like killing Joffrey and Governor in a single person. Im guessing he survives till bastardbowl

That's a very Games of Thronsian response, but I don't think the time is right for him to have killed Ramsay. Act as ambassador to the other houses for him due to privileged position, ensure they're lining up behind you not the Boltons, then move when you're actually ready to strike.
Ramsay is doomed, everyone surely knows that.
