The point of the Fry/Hitchens paraphrase is that the phrase 'I'm offended' shouldn't be allowed to shut down discussion. It's commonly mistaken for a blanket defense of offensiveness, but that's pretty far from the original intent.

There is one and only one reason why you would ever refer to someone by the incorrect gender: because you choose to insult them. You may disagree with transgenderism, it may be contradictory to your beliefs, but this isn't the first time you've have to suck it up and and bite your tongue. As long as you're a person that gives a shit about being a decent human being, you do not go out of your way to disrespect an individual who has done nothing to deserve it. If you care about being polite, you refer to someone as the correct gender. If you want to be an asshole, say whatever the hell you want, and live with the consequences. I agree. Just don't be an asshole to people; be nice. As weird as it may sound, I can't imagine anything transcending the idea of just being polite and decent to someone. :S.
