God, it all goes back to Revival's first attack. Chasing probes around randomly. Targeting one nexus to half hp, than targeting core for a few hits, then spreading everything to get killed by voidrays.
If he'd taken his initial units in and focused the main nexus, while rallying lings to kill third, he gets twice the advantage and crushes free 5 minutes later.

he had mass HT. You won't make it through the storm and whats left over will be cleaned up by void rays, which are not bad in direct combat vs mutas.
