I believe he's talking about the different between sex (which you are referring to and biologically determined) and gender which is arguably self determined.

First time I looked, even when I knew, I was like wow (s)he's hot. But on second pass, her pose and body shape are a bit mannish. It's just that his face kind of draws attention away from the rest.
