People wear name tags sometimes. It's not hard to believe that someone just forgot to take it off. You don't have to wear it all day, just for 5 minutes until he sees you.
I work in a government office building, people don't wear name tags.

My name is Shane, but seriously 2/3 people I meet call me Shawn after they've met me. I have no idea why. I don't know any Shawn or Sean's that get Shane by mistake. It's gotten to the point where I don't even bother correcting people anymore. I just roll with it. I worked with a guy at a job during high school for 2 years and he called me Shawn starting day 1, despite the other 30 people we worked with all calling me Shane in front of him countless times, day in and day out.
This happened to me once when I was in HS. My Football coach kept calling me by my brothers name, he did this off and on for almost two years. My brother is 5 years older than me too, so he wasn't even in HS when I was there. I think the problem is that he taught my brother a lot more than me, so his name was ingrained in his mind.
Honestly, if I were you, I wouldn't ever tell him my real name (assuming he actually doesn't know, and isn't just calling you Claude as a joke). Then you'd have like your own secret identity. People around the office would call you Alzadar like usual, and then you'd have that one special coworker who just calls you Claude for seemingly no reason. Months or even years would go by before he'd notice. Then he'd be all like,
"Your name is Alzadar? Why didn't you tell me I was saying it wrong?"
Then you'd be like,
"Didn't I?"
Then you'd take a long sip out of your coffee mug and nonchalantly go back to work while he just stands there, pondering about his grasp on reality.
They could make MOVIE out of this! Or at least small segment in tv show like Friends.
Honestly though, I agree with people saying don't tell him. If he was at least close to your name then it would be fine to correct him. For example, if you were Brendan and he called you Brennan. That's a simple mistake. But if he is completely off it's because he doesn't give a shit about you, or is completely stupid.
I'm surprised you have let someone call you the incorrect name twice. Personally I would just make a joke out of it and next time he says Claude just say "who is this Claude? I'm Shottaz" but be polite and say "don't worry, mistakes happen sometimes"
People seem to call me Chris a lot, it's not my name. They get a phonecall if they use it in an email.
