Hm... Well, can't really say anything about Berlin, because I'm Swiss and therefore won't ever try to speak English in Berlin. But in Italy I think you could survive without knowing the language. Sweden was absolutely no problem, even in Greece it wasn't too big of a deal if you didn't speak Greek. But in France, people just don't care if you don't understand anything they're saying.
In Japan people don't speak English, but at least they try everything in order to communicate with you :-P Which is at least very friendly :D
