I don't think its someone knew because the foot is all worn out like he/she has recently been in a battle. As for my guess on who it is it is Tsunade. We know she doesn't were shoes just goes around barefoot on sandals. We know that she just had the fight of her life. We know that Orochimaru is done healing her. And ultimately we know she focuses on healing jutsu.

can someone explain how the fourth and kakashi got seperated from naruto? I just find it a little odd that naruto dies and his father is nowhere to be found. are they still pondering what to do with obito? shouldn't they have sense whats happening?

4rth and Kakashi stayed with Obito where he got defeated while everyone else went to where 1rst and Madara were fighting.

Maybe Orochimaru said it that way because he wanted to replicate Kushina's power in Karin when he took her in. Remember that he is the guy with the test subjects and mad research?

It stands to reason that Orochimaru personally witnessed Kushina using them on the battlefield and so he made the connection. Maybe they're first cousins or something, but I think that siblings is impossible. For one thing I think Kushina would have mentioned it to Naruto during their mindscape chat. After a second read through you're right that there's more to it that Oro worked out but I don't think it's that they're siblings.

Not to be argumentative, but if the only two options presented are "use filthy hands to manually keep heart going" and "let him die" then she is most likely going to choose the first.

If the person's hands are small enough, they can slip through. You can also come up from the bottom of the rib cage, where your lungs and stomach normally rest. It's not easy, and pretty difficult to bend the wrist that way, not to mention very painful for the person who literally has a hand stuffed into their chest, but he's unconscious so he wouldn't feel it, and even if it's unsanitary it is emergency triage at it's most extreme.
